Objectiv To investigate parameters and figures of thrombelastogram of coagulation in patients with severe traumatic brain injury(STBI).Methods Three groups were studied:(A)patients with severe isolated traumatic brain injury(n=31).They had no operations.CTs measured when patients were just in hospital supported that they were with severe isolated traumatic brain injury.(B) patients with general trauma without TBI(n=6).(C) healthy volunteers(n=18).TEG parameters of A and B groups were measured in 24 hours,72 hours and a week.And C group as control group only measured one time.The parameters contained R(time of reaction),K(coagulation time),Ma(maximum amplitude of thrombus),α(solidification horn),APTT(activated partial thromboplastin time),TT(thrombin time),PT(prothrombin time),FIB(fibrinogen) and PLT(platelet count).Results The results revealed the hypercoagulabllity was in the patients with severe isolated TBI in 24 hours and the hypocoagulability was in 72 hours.It was statistically significant.It was different between A group and B group and C group.1.here are hypercoagulability in 24hours,but the state of low coagulation in 72hours, and almost normal in a week in A group.2.It is different betweenA and B groups in 3 days but consistent with eath other in a week.Conclusions1.There are obvious coagulation abnormalities in early stage of patients with severe traumatic brain injury.2.We could find a process of coagulation abnormalities of patients with severe traumatic brain injury is from hypercoagulability in 24 hours to the state of low coagulation in 72 hours and then returned to normal in a week.3.TEG Comparison of A group and B group shows that coagulopathy is specific and regularity.4.There is correlativity between Prognosis of the patients with severe isolated TBI and GCS scores.
