Background: Malignant tumor is one of the most important reason of human's death. Gastric cancer is a kind of the most popular human malign- nant tumor,Although with the development and perfection of norm's radical operation of gastric cancer, the local recurrence rate of gastric cancer had descented in recent years ,the death rate is also in the front row of malignant tumor. The metastasis is one of the most important reason of the tumor patients'high death rate,different tumor have their preferred transform style and homing sites. Chemokine is a kind of cytokine superfamily,there are 50's kinds of chemokines as documents reported, and they are all little soluble protein. Many cells can generate chemokines when they actived. As the chemokines combinated with special receptors, they accommodate several kinds of biological function.The interaction of both- side control the immunocyte's orientational migration between the circu- latory systerm and the organ. Accordingly, chemokines and their receptors play an important role in control the cell differentiation,development and orientational migration. According to the different location of the cysteine residue, All these chemokines are devided to 4 subtribe, Namely C,CC,CXC and CX3C,And the corresponding receptors are XCR,CCR,CXCR and CX3C.In recent years, study display that chemokines and the corresponding receptors not only express in immunocyte,but also express in some tumor cells,The ligand of chemokine receptor CCR6 is CCL19 and CCL21, and chemokine CCL20 combines with chemokine receptor CCR7. Both these signal transduction channel play important role in many tumorous disse- mination and organ specificity metastasis.Objective: We try to elucidate the mechanism of the lymph node and peritoneal metastasis of gastric cancer through the detection of chemokine receptor CCR6 and chemokine receptor CCR7 expression in different kinds of specimens of stomach carcinoma.Method: We selected 61 cases of primary lesion from patients with gastric cancer who accepted operations in first affiliated hospital of Dalian Medical University during January, 2008- December, 2008,and collected 11 cases of peritoneal metastasis lesion and 23 cases of paracarcinomatous tissues(far away 6-8cm from the edge of primary lesion). All these cases did not accept radiotherapy or chemotherapy before operation. Immunohisto- chemistry was conducted to detect the expression of chemokine receptor CCR6 and chemokine receptor CCR7 in different kinds of specimens,which were fixed in formalin then embedded in paraffin. The correlation between the expression of CCR6 and CCR7 in primary tumor and clinicopathologic factors was analyzed by univariate analysis . Mutivariate analysis was carried out to evaluate the risk of lymph node metastasis using multi- variate logistic regression model.Results: Immunohistochemistry illustrated that the expression of chemokine receptor CCR6 and chemokine receptor CCR7 was on the membrane or in the cytoplasm of gastric cancer cell. The expression rate of CCR6 in primary tumor,peritoneal metastatic tumor and paracar- cinomatous tissues mucosa cell was 50.8 %,90.9 % and 0.00 % respectively(p<0.05). The expression rate of CCR7 in primary tumor,peritoneal metastatic tumor and paracarcinomatous tissues mucosa cell was 62.6%,100% and 4.3%。The expression of CCR6 in primary tumor was correlated with the tumor stage, depth of tumor invasion and lymph node metastasis (p<0.05), but not with the age of patients, gender, location of tumor, tumor size and pathological differentiation (p<0.05). The expression of CCR7 in primary tumor was correlated with the tumor stage, depth of tumor invasion, pathological differentiation and lymph node metastasis (p<0.05), but not with the age of patients, gender, location of tumor and tumor size (p<0.05).Conclusion: Chemokine receptor CCR6 is expressed in primary lesion and metastic lesion , and no CCR6 expression in normal gastric mucosa,Chemokine receptor CCR7 is expressed in primary lesion and metastic lesion , and few less CCR7 expression in paracarcinomatous tissues mucosa.. CCR6 expression in primary tumor is correlated with tumor stage, invasion depth, lymph node metastasis. CCR7 expression in primary tumor is correlated with tumor stage, invasion depth, pathological differentiation and lymph node metastasis. Chemokine receptor CCR6 and chemokine receptor CCR7 may exert an important role in the progression of gastric cancer and in gastric cancer metastasis ,especially in peritoneal metastasis.
