Objective To study the development of thrombin(TB) following experimental intracerebral hemorrage(ICH) in rats, and analyze the relationship between TB and the apoptosis of brain cells and brain edema in the brain tissue around the hematoma,and then,find out the effects of TB on the apoptosis of brain cells and brain edema around the hematoma.At last,it would be provided that the experimental evidence of removing hematoma for its importance and necessary in the earlier time.Method (1) 105 adult male SD rats were randomly divided into 3 groups:ICH group, physiological saline group and normal contrast group.35 rats of each group were randomly divided into 7 time phases:6h, 12h, 24h, 48h, 3d, 5d and 7d. 50ul of whole blood from itself was stereotactically injected into right globus pallidus of each rat of ICH group to establish a model of ICH,50ul of physiological saline was injected into the same aera of each rat of physiological saline group,and nonthing was injected in normal contrast group.(2) At each phase(6h, 12h, 24h, 48h, 3d, 5d, or 7d) after the rat model was made, Thrombin-anti-Thrombin(TAT) complex in brain tissue aroud the hematoma was determined,using Rat TAT complex ELISA kit;The brain cell apoptosis rate was analyzed by flow cytometry;The brain moisture rate was observed with a dry-wet-weighing technique;The morphological change was observed with electron microscope.In the same way,we observed the brain tissue of physiological saline group and normal contrast group.(3) Then we analyzed the relationship between TB and brain cell apoptosis ,and brain moisture rate ,using SPSS 13.0.Results (1) After 6 hours of ICH model finished,the concentration of TAT aroud hematoma of ICH group was obviously higher than that of physiological saline group and normal contrast group(P<0.05);at the phase of 48 hours ,it reached its maximum(P<0.01); 3 days later, it began reducing(P<0.01);at the phase of 7days,it was still higher than the other two groups(P<0.01).there was no diference between the other two groups,and between every two phases in either of the other two groups(P>0.05).(2) After 6 hours of ICH model finished,the apoptosis rate of brain cells aroud hematoma of ICH group was obviously higher than that of the other two groups (P<0.05);at the phase of 48 hours ,it reached its maximum(P<0.01); 3 days later, it began reducing(P<0.01);at the phase of 7days,it was still higher than the other two groups(P<0.01).there was no diference between the other two group,and between every two phases in either of the other two groups (P>0.05).(3) After 6 hours of ICH model finished, the brain moisture rate aroud hematoma of ICH group was obviously higher than that of the other two groups(P<0.05); 3 days later, it got to its maximum(P<0.01);at the phase of 7 days,it began going down(P<0.01) ,and was still remarkably higher than the other two groups(P<0.01).there was no diference between the other two groups,and between every two phases in either of the other two groups (P>0.05).(4)There was a significant correlation between the concentration of TAT and the apoptosis rate of brain cells,and the brain moisture rate in ICH group(r:0.627,0.315; both of P <0.01).(5)Using electron microscope,we observed brain cells apoptosis , remarkable edema aroud capillary vessel,the tissue clearance which was obviously wider than the normal tissue, swelling chondriosome of brain cells and nerve fiber demyelination ;These changes in ICH group were obviously more than the other two groups.Conclusions (1) Following ICH , the concentration of TB,the apoptosis rate of brain cells, and the brain moisture rate aroud hematoma went up and exhibited with time-depended change, and could go on beyond one week;(2) the concentration of TB aroud hematoma was significantly correlated with the apoptosis rate of brain cells, and the brain moisture rate;it was proved that TB may result in secondary brain injury after ICH,such as brain cells apoptosis,brain edema(vasogenic brain edema) and so on;(3) If the hematoma were removed earlier , it may be improved that the problem of secondary brain injury result from TB.
