OBJECTIVE:To explore the expression of NCAM and OCT-4 in the cryptor-chidism and its'clinical significance.MATERIALS AND METHODS:1.This study comprises 50 specimens from 48 cryptorchid boys(41 unilateral and 7 bilateral) who underwent orchidopexy with simultaneous testicular biopsy at Children's Hospital of Chongqing Medical University between September 2004 and April 2005,41 specimens from 41 unilateral,4 specimens from 2 bilateral and 5 specimens from 5 bilateral. Among them,there were 10 at the internal ring,21 within the inguinal canal,14 at the external ring,just 5 above the upper scrotum in a superficial inguinal pouch.Tissues were preserved in liquid nitrogen.2.Frozen section for Immunohistochemistry for NCAM and OCT-4 in 50 specimens,using Histostain-SP kits.3.The statistics on expression of NCAM and OCT-4 was made.The relations between position,age and the expression of NCAM and OCT-4 were also analysised with statistical methods.RESULTS:1.The NCAM is expressed in seminiferous tubule in 15 of 50(30%),including 13 of 50(26%) positive and 2 of 50(4%) negative in interstitial cell.The total negative in interstitial cell are 16 of 50(32%).2.The relation of age of surgery and NCAM positive in seminiferous tubule.They were divided into<1.6-year group,1.6-5 year group and>5 year group.We find that 2 of 9(22.2%) speimens was under 1.6-year-old,6 of 27(22.2%) speimens were fom 1.6 to 5-year-old,7 of 13(53.8%) speimens were older than 5-year-old.There was no statistical significance between groups(P>0.05),but obviously the expression in the oldest stage inreases following times.3.The relation of testicular location and NCAM positive in seminiferous tubule.Testicular locations were within the inguinal canal in 7 of 21(33.3%) specimens,at the external ring in 4 of 14(28.6%) specimens,at the inte nal ring in 2 of 10(20%) specimens,just above the upper scrotum in a superficial inguinal pouch in 2 of 5(40%) specimens. NCAM positive in seminiferous tubule has no significant differences in different testicular locations(P>0.05).4.The relation of age of surgery and NCAM negative in interstitial cell.We find that 7 of 9(77.8%) specimens was under 1.6-year-old,8 of 27(29.6%) speimens were fom 1.6 to 5-year-old,1 of 13(7.7%) speimens were older than 5-year-old.There was statistical significance between groups(P<0.05),but only between the flrsr stage and the third stage(P<0.017).5.The relation of testicular location and NCAM negative in interstitial cell.Testicular locations were within the inguinal canal in 6 of 21(28.6%) specimens,at the external ring in 6 of 14(42.9%) specimens,at the internal ring in 3 of 10(30%) specimens,just above the upper scrotum in a superficial inguinal pouch in 1 of 5(20%) specimens.NCAM negative in interstitial cell has no significant differences in different testicular locations(P>0.05).6.The OCT-4 is expressed in 6 of 50(12%),including 4 specimens with NCAM positive in the seminiferous tubule.They all have no regularity in the age of surgery or in the testicular locations.7.The pathological expression of NCAM and OCT-4 in seminiferous tubule are 17 of 50(34%):NCAM(+)OCT-4(-) 11 of 50(22%),NCAM(+) OCT-4(+) 4 of 50(8%),NCAM(-)OCT-4(+) 2 of 50(4%)8.The nomal expression of NCAM and OCT-4 in the cryptorchidism are 38%(19/50),NCAM and OCT-4 negative in seminiferous tubule,and NCAM positive in interstitial cell.CONCLUSIONS:1.In the cryptorchidism,there are pathological expression of NCAM and OCT-4:Afer 3 months of age,NCAM positive in seminiferous tubule,or NCAM negitive in interstitial cell or OCT-4 positive in seminiferous tubule.2.Using immunohistochemical expression of NCAM and OCT-4,If NCAM or OCT-4 is positive in the seminiferous tubule,the migration and differentiation of gonocytes may be defective or delayed,a risk of subsequent fertility potential and canceration for cryptorchidism.3.If the expression of NCAM is negitive in interstitial cell,the development of Leydig's cells in undescended testis may be delayed.4.In clinical,for cryptorchidism,we can estimate the infertility potential and canceration,using immunohistochemical expression of NCAM and OCT-4...
