Objective: The aims of this study were to analysis high risk human papillomavirus (hrHPV) infection in female cervix, explore the dose-response relationship between hrHPV virus load and cervical lesions and the relationship between hrHPV virus load and the severity of cervical lesions, provide scientific basis for the primary and secondary prevention of cervical cancer.Methods: Cytological samples of cervix were detetced by hybrid capture2 (HC-2) system for the virus load of HPV-DNA. HrHPV viral load (expressed as RLU/CO) was divided into five groups: <1, 1-, 10-, 100—, 1000-10000. According to the severity and range of the heteromorphic cells, the subjects were classified as negative or chronic inflammation, cervical intraepithelial neoplasia (CIN) 1, CIN2, CIN3 or carcinoma in situ, or cervical cancer. Demographic information, gynecological history and the treatment history, the results of hrHPV detection Biopsy were collected. Mann-Whitney test were performed for the measurement data, X2 test was used for the statistical test of the enumenration data. Mantel trend analysis was used to analyse the dose-response relationship between hrHPV virus load and cervical lesion. Spearman's rank correlation was used to analyze orderly multi-categorical data.Results:1.The positive detection of 26294 subjects was 26.74%. Of the hrHPV infected women, 30.45% of whom virus load were distributed between 100-1000 RLU/CO.2. There was significant difference of hrHPV infections in different age goups, <25 years age group got the highest infection rate (33.63%).3. Each age group got the highest hrHPV infection rate in the Pearl River Delta area (Guangzhou, Dongguan, Huizhou).4. A total of 861 cases who both received hrHPV test and the Biopsy. Four hundred and fifty four cases were hrHPV positive, giving an infection rate of 52.73%. The hrHPV infection rate differed according to histology and grade. Most hrHPV infecton cases were in the 10-100 RLU/CO or 100-1000 RLU/CO virus load grades, the constituent ratio of which were 27.49% and 34.37, respectively.5. The cervical lesions rate increased as the age growth. Curve estimation showed good correlation between cervical lesions rates and hrHPV load (r=0.775,p<0.05); the rate of cervical lesions increased with hrHPV load (Xtrend=8.000,p<0.001).6. There was line trend between the hrHPV virus load and the severity of cervical lesions; positive intensity rank correlation was found between hrHPV load and severity of cervical lesions, and a correlation appeared between hrHPV load and severity of cervical lesions.Conclusion:1. The hrHPV positive detection rate of the sample from Guangzhou Kingmed Center for Clinical Laboratory was relative lower than other areas in China, the Pearl River Delta area got a higher hrHPV positive detection than the other areas in Guangdong province.2. There was a dose-response relationship between the level of hrHPV load and cervical lesions in those older than 35 years women, which cervical hrHPV infection has a correlation with CIN2 or greater abnormality of cervical lesions. 3. There was a relationship between the hrHPV virus load and the severity of cervical lesions in those older than 35 years women, which suggest the hrHPV load has a value in predicting the severity of cervical lesions.4. HC-2 system has important clinical applications in cervical prevention.
