The Clinic Value Of The Collateral Vessels Of Portal Hypertension

Objectives:To study the different collateral veins of the portal hypertension by multi-slice spiral CT portography,and investigate the clinical value of the collateral vein.Materials and Methods:64 slices computed tomography was performed in the all 102 patients to overview the collateral vein of the portal vein.In the 102 portal hypertension patients,there were 70 cases with upper gastrointestinal hemorrhage,64 cases with ascites and 4 cases with hepatic encephalopathy.Collect the history of the patients and take the PLT,ALB,PT test,then determine the width of portal vein and the thickness of spleen by color Doppler sonograph.The results were analyzed by SPSS 11.0.Result:In the MSCT portography examination of 102 patients with p ortal hypertension,there were 91 cases with esophagus varix,75 case s with gastric varix,24 cases merely esophagus varix,67 cases with gastroesophageal varix,8 cases with isolated gastric fundus varix.The blood supply of the varices was the left gastric vein in 79 cases,th e left gastric vein and the short gastric vein or the posterior gastric vein in 15 cases,all of the three veins in 5 cases.The MSCT portog raphy also showed the gastro-renal vein in 9 cases,splenorenal vein i n 8 cases,umbilical vein and abdominal wall vein in 19 cases,retrop eritoneal-paravertebral vein in 8 cases,cavernous transformation of po rtal vein in 7 cases,embolus of portal vein in 15 cases.The result o f statistical treatment shows that the collateral vein has a close relati onship with the clinical feature of the portal hypertension.The bloodi ng rate of varices with two or more than two supply veins is higher than varices with only one supply vein.The gastro-renal vein can red uce the incidence rate of ascites.Conclusion:The MSCT portography was a useful diagnosis tools for portal hypertension,and it could display the courser and disposition of the collateral vein of portal vein clearly.The formation of the collateral vein has a good value in prognose the clinical feature of portal hypertension.
