Objective:To investigate the characters of caesarean delivery during the second stage labor,the impact on mothers and neonate,as well as how to reduce the rate of caesarean delivery during the second stage labor; to explore the application of antibiotics.Methods:1.Used the 69 cases caesarean delivery during the second stage labor in the first affiliated hospital of Dalian Medical University from Jan.1999 to Dec.2008 as study group,randomly selected 70 patients with caesarean delivery during the first stage labor as a group for comparison,retrospective analysis of two groups of abnormal labor,height of fetal head before surgery,surgical indications,time and complications,amniotic fluid and neonatal apgar score traits.2.To combine study and control groups cases,divided according to the usage of antibiotics-alone or combined,to observe the two groups of patients with fever,to investigate weather the combination of antibiotics is better than the antibiotics alone.Results:1.The study of the research group and the group comparison showed:in protracted active phase,prolonged active phase and protracted descent,a significant difference between the two groups (p<0.05);in the surgical indications,abnormal presentation position had a statistically significant difference (p<0.05);as for surgery complications,in the aspects of difficulties in delivery fetal head,extension of uterine incision split, manual removal placenta and hematuria,the two groups showed statistically significant (p<0.05);amniotic fluid and neonatal apgar score traits had no statistically differences (p>0.05).2.For the prevention of postoperative fever,joint use of antibiotics compared the use of antibiotics alone,there was no significant difference (p>0.05).Conclusions: 1. Methods of reducing the second stage of labor cesarean section:1)pay attention to the abnormality of the first stage of labor,particularly emphasis on abnormal drop of fetal presentation.A higher fetal presentation (≤S+2)at the end of active phase or in the second stage labor might predict dystocia.2) combining the usage of partogram and score of cephalic position delivery to observe the progress of labor.2. The caesarean delivery during the second stage labor may lead to the extension of uterine incision split,difficulties in delivery fetal head,hematuria after surgery,it should be avoided.3.For the prevention of postoperative fever , applying antibiotics jointly is not better than applying solo.
