| Objective:To evaluate the roles of angiocardiography(ACG) and Multi-slice spiral computed tomography(MSCT) in the diagnosis of Complex congenital heart dieases(CHD).Methods:82 Patients with complex CHD confirmed by operation were analyzed respectively,including 40 cases of boys and 42 cases of girls.Patients'age ranged from 3 months to 14 years(mean age 5.2±2 y).All of them were examined by two- dimensional echocardio- graphy. 44 cases of them were examined by ACG and 64-slice spiral CT as well. In addition,38 patients with CHD complicated with pulmonary arteral hypertention(PH) were studied.18 of them underwent surgery,the others did not.And pulmonary artery pressure and total pulmonary resistance were measured by cardiac catheter preopera-tively in all of them,64-slice spiral CT was used in the 10 cases of them.Results:(1)The accurate rate of 64-slice spiral CT and ACG in diagnosing complex CHD was 86.55%(103/119)and 94.96%(113/119) there were signifcant difference(P<0.05) respectively.Accuracy rate could up to 99.16%(118/119) when both were combined,there were signifcant difference(P<0.05).(2)Accuracy rates of intracardiac malformation were 81.08%(60/74)and95.95%(71/74),There were significant difference(P<0.05).while those of extracardiac malformation were 95.56%(43/45)and93.33%(42/45) by MSCT and ACG,,There were no significant difference(p>0.05)(3)In the study of all cases of Complex CHD complicated with pulmonary arteral hypertention,the results showed that total pulmonary resistance(≤15wood)in surgery group,which was much lower than that in non-surgery group(≥17.3 Wood),and pulmonary artery pressure of the former(≤66 mmHg ) was lower than that of non-surgery group(≥66 mmHg),There were significant difference(P<0.05).Conclusion:ACG is superior to MSCT in the detection of pediatric complex CHD.The combination of MSCT with ACG can raise the definited diagnosis rate,especially for the intracardiac malformation.But the ACG was should used to measure the pulmonary artery pressure and total pulmonary resistance for the complex CHD with PH. |
