Objective: To study the factors of influencing the recovery of oculomotor nerve palsy caused by posterior communicating artery aneurysm after surgical treatment, the clinical features,the time of microsurgery,the methods of microneurosurgery and postoperative treatment.Methods: The clinical data from 34 patients with ONP cased by posterior communicating artery aneurysm during 2005-5 to 2008-8, was analysed by means of the chi-square test analysis model SPSS13.0. The following factors were evaluated: gender, age, history of hypertension, subarachnoid hemorrhage, size of aneurysm, projection of aneurysm, CT-fisher grade, Hunt-Hess scale, the degree of preoperative oculomotor nerve palsy, the time between the onset of palsy and the time of surgery, the side of the aneurysm, the surgical approach. Whether the aneurysm was ruptured, the parent artery of aneurysm accepted temporary break in the operation, the sac decompressed, papaverine used in the operative region, postoperative neurotrophic treatment or not. Results: In 34 patients, 19 patients had completely recovery of ONP; 14 patients had partial recovery; 1 patient had not made any improvement. In conclusion, the factors influencing recovery of ONP were found to be the degree of preoperative oculomotor nerve palsy, and the time between the onset of palsy and the time of surgery. Other factors did not haveeffect on the recovery of ONP in our series.Conclusion: 1. The total effective recovery rate of aneurysmal ONP was 97.1 % in our group.2. The degree of preoperative ONP,the time between the onset of palsy and the time of surgery had effect on the recovery of ONP. Whileother factors had no effect on the recovery of ONP in our series.3. A good recovery of ONP after surgery happened in the patients with partial preoperative ONP; the patients with complete preoperative ONP and the time of duration of palsy was longer than 14 days, would have a poor recovery of ONP.4. Early surgical treatment was associated with more favorable recovery of ONP.
