Influence Of One-lung Ventilation On Respiratory Function During Perioperative Period Of Esophagus Cancer

Objective One- lung ventilation is commonly used in radical operation of esophageal cancer as an anesthetic technology.To study the effect of OLV for esophagectomy in the treatment of esophageal cancer patients is the purpose in this study.Method 60 cases were divided into 3 groups(n=20) randomly. The patients in Group A underwent two-lung ventilation in the whole course,Group B underwent one-lung ventilation intermittently,and Group C underwent one-lung ventilation in whole course.Monitoring vital signs and airway pressure routinely.Blood gas analysis was performed in every case around the whole preoperative period. Preoperative and postoperative specimens of lung tissues were submitted to examine morphological changes.The operation time was recorded and the intraoperative condition of lung was observed.Results Operative time of Group B,C were shortened by down to 26.4%(P<0.05),27.2%(P<0.05) compared with Group A,while there was no significant differences between Group B and C(P>0.05).Three groups' value of pH,PaO2 and PaCO2 fluctuated around the normal range. Airway pressure of Group B and Group C during one-lung ventilation escalated significantly(P<0.05),but recovered to preoperative level.The lung was found engorged seriously before the closure of the chest in Group A,but not in Group B,C.To compare preoperative and postoperative lung water content,only contrast intragroup of Group A was statistically significant(P<0.05).The cases' degree of congestion and edema under light microscope level,were better in Group B,C than that in Group A.Under electron microscope level,cell structures were damaged in all three groups,but Group C was slighter.Conclusions 1.The patients undergoing one - lung ventilation intermittently for esophagectomy had a better result compared with that undergoing one - lung ventilation or two - lung ventilation alone and that is conducive to restoration.2.One - lung ventilation intermittently will not affect operating.
