Objective: To evaluate the role of vascular endothelial growth factor (VEGF) in scar formation of the filtering bleb, we observed the expression of VEGF in the filtering bleb after various various combined trabeculectomy in a rabbit eye high intraocular pressure model.Methods: First, we took 30 rabbits randomly. Trabeculectomy was practiced on left eye, while the untreated right eye was considered as control group. Second, on the other 30 rabbits, trabeculectomy plus mitomycin C (MMC) was performed on left eye as MMC group, while trabeculectomy with amniotic membrane transplantation on right eye as amniotic membrane group.On postoperative day 1, 3, 7, 14, 21 and 28, intraocular pressure, filtering bleb appearance, cornea and fundus were observed. In addition, on postoperative day 7, 14, 21 and 28, the rabbits were killed in batch and tissues were harvested from the filtering bleb area. Expression of VEGF was examined by immunohistochemical staining.Results (1) Very low VEGF was expressed in control group. At the same timepoint, expression of VEGF in trabeculectomy group was significantly higher than that of control group (P<0.05). While no significant differences of the VEGF expression level was observed between MMC group and amniotic membrane group (P>0.05) . (2) On postoperative 1 day and 3 day, the expression of VEGF was significantly upregulated, peaked prominently on 7 day, and then decreased on 14 day. On postoperative 28 day, VEGF level drew back to 1 day (P>0.05). (3) VEGF level in MMC group and amniotic membrane group was dramatically lower than trabeculectomy group on postoperative 3, 7 14th and 21 day.Conclusion VEGF plays an important role in the wound healing of the filtering bleb after trabeculectomy. Inhibition of VEGF synthesis or its activation may help to prevent scar formation of glaucoma filtering surgery.
