Compensating Analysis Of New Rural Cooperative Medical Scheme(NCMS) In Kuancheng District Of Changchun

The new rural cooperative medical scheme (NCMS) is a farmer medical service cooperation institution, based on comprehensive arrangement for serious disease, which is organized, guided and supported by government with farmers'voluntarily participation, and is financed by individuals, the collective, the government and so on. This work must be carried out with the principles of voluntary, cooperation, public and service. NCMS is an important Institutional Innovation in the development of Rural Health Reform, an important realization of farmer basic medicine insurance in our country at the present stage, and a concentrated expression of policies to support and benefit agriculture, rural areas and farmers at the new period.The relevant policies in our country stipulated that NCMS is a financial mechanism carried out in a combination of individual payment, collective support and government subsidize. As a new one of policies to support and benefit agriculture, rural areas and farmers supported by our party and country at the new period, NCMS is facing a series of problems. First of all, when the substandard problems in local government's financial subsidy will be realized successfully in the central authorities'financial subsidy, the problems of local government's financial subsidy realization lie in what the standard of financial subsidy in NCMS stipulated by central authorities for local government is, and how local governments, three levels of organizations including province, city (area) and county, divide the specific financial burden to take. Moreover, the unreached local government financial subsidy or lending behavior in the case of reached financial subsidy will take the negative effects into NCMS, and lead to the emerge of subsidy interruption, delayed, decrease in the degree of farmer benefits, and so forth. For a long time, the complex procedures of compensation in NCMS and the high cost of Intermediated Financing are always the more concentrated problems complained by workers who participate the work of farmer and basal NCMS. The prevalence ways of Intermediated Financing, in most of city or country participating NCMS, is to charge fee from door to door. Although it is exciting that the level of Intermediated Financing is increasing, the increase still definitely has trace ruled by men, and the mechanism of stable increase in financing has not been established. Lake of this, the guaranteed effect could not be exerted as one of institutional assurance. Therefore, according to medical expenses, government receipts and growth of farmers'income, it is a buring work to establish the schema of increase in financing in NCMS.As one of state keystone touching provinces, Jilin province actively yet prudently develops the construction of NCMS, and has covered the whole province until 2007. To sustain the NCMS, Jilin Province further strengthened the construction of rural health delivery system, improved sanitary condition and elevated the serviceability; and it has established the mechanisms of stable input and adequate financial resources, improved the conditions of medical equipment in villages and towns public health center, and stabilized the troop of community healthy professionals.This paper analyzed the information in 2008 of reimbursement compensation for farmers who are co-participaters in NCMS, including the benefit of co-participation of the farmers in hospital, the range of all co-participation farmers'age in hospital, the range of all co-participation farmers'gender in hospital, the distribution of compensation at all levels of hospital, the hospital-quarter reimbursement comparison of treatment in outside hospital in different economic levels of areas, the statistics of non-reimbursement at all levels of hospitals'cost, and the condition of out-patient benefit from the participation of the farmers in kuancheng district in changchun in 2008.Methods: (1) we generally analyzed the co-reference information of NCMS provided by the office about 2008 kuancheng district and the farmers'compensation of hospitalization in different medical institutions at all levels of the year. (2) We summarized, analyzed and compared these data of different ways about compensation for hospital, outpatient reimbursement of compensation payments. (3) Comparative analysis of pairs of different ages, different gender in treatment staffs (4) analysis of levels of reimbursement for hospital outside the region such as attending at different economic areas.Taking indicators of operability, sensitivity and representation into account, we screened the original by use of expert advice method and discrete indicators of trends in method. Excel database was used for data entry. Spss12.0 statistical software was used for data analysis Results: 1.The basic conditions of NCMS is running well and on the right track in 2008 years, the enthusiasm of the farmers a higher co-argument runs high, and the majority of farmers'basic health care has been guaranteed. The rate of participation positively correlates with levels of economic development. 2. The funds of NCMS runs in good condition, can keep balance between income and expenditure in kuancheng district.3. There is significant difference in co-farmers'choices to the designated medical institutions in kuancheng district, resulting in the unreasonable waste of medical resources; the older the patients of co-farmers, the higher the proportion of being sick in hospital. The number of women hospitalized and composition is much higher than the ones of males. 4. The higher-level of hospitals, the more hospital admissions, accordingly, the more total hospital charges, hospital days and hospital charges. 5. According to the practical compensation efficiency, it is necessary to compensate for future work to improve out-patient approach in kuancheng district.The sustainable development issues of NCMS in kuancheng district: the top leaders of district must take on the leadership personally, strengthen the responsibility, improve the mechanisms, and effectively solve the funding shortage. Designated medical institutions in areas of service should be strengthened, the level of village-level health care institution-building and health service quality should be improved, and the sales channels of drug must be standardized. It is an inevitable trend of development and requirements to establish and improve computer information management system, as soon as possible, to realize the office computer information network management. Rational design of compensation programs and compensation ratio maintain the fairness of health care, improve the enthusiasm of farmers taking apart in the reasonable control of insured medical costs, and strengthen the fund management and supervision.
