| Objective TO compare the diference of microalbuminuria(MAU) and serum high molecular weight-adiponectin(HMW-ADP) between essential hypertensives and hypertensive patients with metabolic syndrome(MS).In order to investigate the association of MAU and HMW-ADP of hypertensive patients with MS, and the correlation between HMW-ADP and the components of hypertension.Methods 68 patients with essential hypertensives were enrolled and divided into two groups by the definition of MS, including the group of hypertensive patients with MS (EH-MS group,n=33) and the group hypertensive patients (EH group,n=35).According to the definition made by International Diabetes Federation ,the diaganosis of metabolic syndrome is central obesity(waist circumference,men≥90cm,women≥80cm) plus any two:①Raised triglycerides (TG)≥150 mg/dL (1.7 mmol/L) or specific treatment for this lipid abnormality.②Low level of high density lipoprotein cholesterol(HDL-C):<40 mg/dL(1.03 mmol/L) in men,<50 mg/dL (1.29 mmol/L) in women,or specific treatment for this lipid abnormality.③Raised blood pressure:Systolic≥130 mmHg and(or) Diastolic≥85 mmHg,or treatment of previously diagnosed hypertension.④Raised fasting plasma glucose:Fasting plasma glucose (FPG)≥100 mg/dL (5.6 mmol/L),or previously diagnosed type 2 diabetes. The sex, age, height, body mass and waist of all subjects were recorded.The urine albumin-to-creatinine ratio (ACR) was measured by radioimmunoassa.The level of serum HMW-ADP was determined by a sandwich ELISA.Results (1) Body mass index (BMI), Waist circumference(WC), FPG, TG, low density lipoprotein cholesterol(LDL-C),uric acid (UA) and MAU were increased(P<0.05orP<0.01,respectively),while HDL-C and HMW-ADP was decreased(P<0.01) in EH-MS group,as compared with those in EH group.(2) Pearson correlation analysis manifested that the serum HMW-ADP was not corelated with ACR (P>0.05).The serum HMW-ADP was negatively correlated with BMI,WC,SBP,PP,FPG and LDL-C(P<0.05 or<0.01),and it was positively correlated with HDL-C (P<0.01)(3) After multiple regression analysis, the LDL-C seemed to be the best variable in predicting HMW-ADP levels.(4) Binary Logistic regression showed FPG,HDL-C,TG,LDL-C,ACR and HMW-ADP were associated with the development of MS in hypertensive patients (P<0.05~0.01).Conclusion (1) MS aggravates kidney damage of hypertensive patients.(2) The serum HMW-ADP was not corelated with ACR.(3) There is a significant correlation aong HMW-ADP and the components of hypertension.LDL-C is the main factor that effected the serum HMW-ADP levels.(4) Increase of ACR and decrease of HMW-ADP promoted development of MS in patients with hypertension. |
