Objection:To study the clinical value of mesothelium-count in differential diagnosis of malignant and tubercular pleural effusion.Material and methods1.General condition of the patients: 132 cases of hospitalized patients with pleural effusion in the Department of Respiratory Medicine, Shandong Provincial Hospital from January 2009 to January 2010.①57 patients with malignant pleural effusion, including 4 cases of malignant pleural mesothelioma.②75 cases of benign pleural effusion, in which 51 patients with tuberculous pleurisy and 24 patients with other benign pleural effusion, including 15 cases of cardiac pleural effusion, empyema in 4 cases, 3 cases of viral pleurisy, parapneumonic effusions in 2 cases.2 .Diagnosis Basis:the diagnosis basis of malignant pleural effusion are the original clinical manifestations, biochemical, imaging examination, and pleural fluid cytology, and (or) pleural histopathological diagnosis, with thoracoscopic biopsy in 23 cases.The diagnosis basis of tuberculous pleural effusion are primary clinical manifestations,biochemical, imaging, and pleural fluid bacteriological examination, pleural biopsy, or through significant improvement in anti-tuberculosis treatment and were followed up for more than six months,with thoracoscopic biopsy in 19 cases.3 .Detection Method: Conventional chest puncture(Little or encapsulated pleural ABSTRACT. centrifuge 5min, suction to the supernatant, take the bottom sediments 1一2 episodes in the load on the slide, pushing pieces smear until the slide dry, Wright Dyeing 15~20min, estimated at the amount of mesothelial cells under a microscope (in "no", "small", "more"," numerous" means).4. Statistical methods: The comparisons of multiple samples and the two samples are tested with the"rank test"and the whole data is processed by SAS 9.2 statistical software.ResultConclusionEpithelial cells in malignant pleural effusion are more ,and in tuberculous pleural effusion are few, most disappeared, and in the other benign pleural effusion are less.So the distributions of mesothelial cells in malignant, tubercular and other benign pleural effusion are statistically different, P<0.05.
