A Study On BISAP In Predicting Outcome Of Patients With Acute Pancreatitis

ObjectiveTo evaluate the value of Bedside index for severity in acute pancreatitis(BISAP) in predicting the prognosis of patients with acute pancreatitis(AP), compare BISAP with Balthazar CT severity index (CTSI), analyze its advantages and disadvantages, so as to provide evidence for applying the scoring system to guide clinical judgement and treatment.MethodsOne hundred and fifteen patients with AP, admitted into Jinan Central Hospital Affiliated to Shandong University from 2007 to 2009 were retrospectively studied with BISAP and CTSI scoring systems. The correlation between the two scoring systems and severity of AP, duration of fasting and hospitalization and fatality rate were analyzed. The values of BISAP and CTSI in predicting severity and complications of AP were analyzed using receiver operating characteristic (ROC) curves.ResultsAmong 115 patients, mild pancreatitis (MAP) was identified in 49 patients and severe pancreatitis (SAP) in 66 patients. The mean scores of BISAP scoring system between MAP and SAP were different significantly (P<0.05). The scores of BISAP and CTSI were correlated significantly with days of fasting and hospitalization. Furthermore, the scores of BISAP were correlated significantly with fatality rate. BISAP system had a similar ability in predicting AP severity to that of CTSI scoring system, with the area under ROC curve (AUC) of 0.771. BISAP had a higher ability in predicting systemic complications but showed a lower ability in predicting local complications compared with CTSI scoring system.ConclusionBISAP is a simple and useful scoring system in predicting AP severity and prognosis. The predictive validity can be improved if combining it with CTSI scoring system.
