Objective:To explore the epidemiological features of chronic idiopathic urticaria in Jinan, and to assess the significance of routine laboratory examinations and autologous serum skin test in the diagnosis. To understand pathogenesis and clinical features of chronic autoimmune urticaria, and to predict prognosis and direct treatment.Methods:Following a certain standard, 60 CIU patients were selected. The general situation and medical history of the patients were inquiried to complete a survey questionnaire. Peripheral blood eosinophil counts, serum anti-thyroglobulin antibody determination and serum total IgE were determined. Autologous serum skin test were acted on every patient. We compared epidemiological features and laboratory examinations and curative effect of ASST-positive group with the negative group. SPSS13.0 software was used for sorting out the data.Results:60 patients,15 males and 45 females. The ratio of male to female was 1:3.41 patients were indoor workers, and 19 cases were outdoor workers.39 cases were urban residents and 21 rural residents. The patients are enrolled with an age range of 12 to 68 (average 39.82±14.63). The largest percentage of total number (53.3%) is from 30 to 50. ASST was positive in 51.7%(31/60)of CIU patients. Comparing with ASST-negative group, positive group probably had a greater number of wheal (P<0.05), more severe pruritus (P<0.05) and longer time (P<0.05) but the larger size of the wheal (P>0.05). The peripheral blood eosinophils counts was determined in 45 patients.1 cases in 23 ASST-positive patients (4.3%)were significantly increased, while 4.5%(1/21) of ASST-negative were elevated. The serum total IgE were determined in 45 patients. 6 cases in 22 ASST-positive patients(27.3%) were significantly increased, while 18.2%(4/22) of ASST-negative were elevated. Anti-thyroglobulin antibodies were determined in 43 patients. 7 cases in 22 ASST-positive patients (31.8%)were significantly increased, while 4.8%(1/21) of ASST-negative were elevated. All of the patients were treated by the second generation antihistamines drugs. The curative effect was evaluated when the patients were treated for 2 weeks and 4 weeks. 24 of 31 cases ASST-positive had a significant curative effect and the remaining 7 cases showed a significant curative effect after immunosuppressants Tripterygium Wilfordii was combined. 28 of 29 cases with ASST-negative had a significant curative effect and 1 case showed an unsatisfactory result even after Tripterygium Wilfordii was combined. Static duration of the blood, position for skin test, dermograph reaction and proficiency of the operator affected the results of ASST during the experiment.Conclusion:1. The morbidity of the chronic idiopathic urticaria were higher in the female than male and higher in indoor workers than outdoor works and higher in urban residents than rural residents in Jinan.2. ASST was positive in 51.7% of CIU patients in Jinan and it is identity with reported data.3. Comparing with ASST-negative group, positive group probably had a greater number of wheal, more severe pruritus and longer time. 4. ASST-positive group may be associated with autoimmune thyroid disease.5. The second generation antihistamines combinated with immunosuppressants Tripterygium Wilfordii had an good curative effect.6. Static Duration of the blood, position for skin test, dermograph reaction and proficiency of the operator can affect the results of ASST.
