Objective:Colorectal cancer is still one of the world's most common malignancy, early colorectal cancer 5-year survival rate after surgery can be as high as 90%, while the late less than 10%. Therefore, early diagnosis and increase the rate of colorectal cancer to become the key to survival. At present, surgical treatment still the main treatment for colorectal cancer, preoperative evaluation to determine the correct treatment and prognosis. Past examination of colorectal disease, rely mainly on conventional colonoscopy and digestive tract angiography, the accuracy was high, but there are many patients can not tolerate or obstruction due to circumstances check fails, and thus the diagnosis and treatment of patients with a greater impact. 64-slice spiral CT three-dimensional imaging the colon can make up for deficiencies in the above-mentioned two kinds of checks. Through the reconstruction of tumor blood vessels and measured the volume density of tumor blood vessels, its pathology and postoperative explore the value of tumor blood vessel density in colorectal cancer invasion and metastasis of role in the process and their mutual relations, research vessel volume reconstruction of colorectal cancer vascular density used in preoperative evaluation of colorectal cancer is feasible. The purpose of this study 64-slice spiral CT three-dimensional imaging of the colon in the Diagnosis of colorectal cancer and colorectal cancer vascular volume reconstruction of blood vessel density values used in preoperative evaluation of colorectal cancer is feasible.Methods:Gastrointestinal surgery in our hospital from October 2008 to 2010, three menstrual surgery and postoperative pathology confirmed 40 cases of colorectal cancer patients before surgery at the same time line of 64-slice spiral CT three-dimensional imaging and fiber colon colonoscopy, in which 22 routine colorectal tumor blood vessel volume reconstruction and measurement obtaining the blood vessel density. 40 patients were male in 26 cases, female 14 cases, male to female ratio 1.86:1, with a median age of 55 years of age (35 years old ~ 75 years). Check the night before last oral administration of 25% magnesium sulfate 150ml (morning, 75 ml, late 75 ml), and a large number of drinking water (1 000 ~ 2 000 ml), fasting to check. 5 ~ 10 min before scanning intramuscular injection of 6542 10 mg. The above information is separate multi-planar reconstruction (MPR), 360°large view anatomical view, virtual endoscopy (CTVE), surface shaded display (SSD), Transparency (RaySum) images, volume rendering (VR), and panoramic endoscopic show (PE) and other three-dimensional reconstruction. By two radiologists and physicians in case I do not know the results of colonoscopy for diagnosis. Volume reconstruction of tumor blood vessels in which blood vessel density values measured by 1000 high-resolution color graphics pathological analysis system (Ji Medical University, 1000 screen the works produced) to complete.Results:Pathological examination confirmed that 40 patients, 64-slice spiral CT three-dimensional imaging the colon diagnosed 38 cases of successful colorectal cancer, the accuracy of 95%. 3 cases were due to intestinal obstruction or stenosis can not lead to colonoscopy, colonoscopy does not reach the ileocecal only line in the colon endoscopic biopsy, 1 case of bowel preparation because of their prime line, it conducted a 64-slice spiral CT three-dimensional colonic imaging examination. 2 cases of colon cancer because of age, infirmity, can not be tolerated CC, but all the successful line of 64-slice spiral CT three-dimensional imaging the colon examination. 64-slice spiral CT three- dimensional imaging examination of the entire colon profile, tumor infiltration and parenteral to conduct a comprehensive assessment of the transfer, effectively compensate for lack of fiber colonoscopy. 22 routine colorectal tumor blood vessel volume reconstruction of vascular density determination of colorectal cancer patients with pathological contrast shows that: Colorectal cancer can be high or low blood vessel density as a predictor of lymph node metastasis and distant metastasis, one of the important indicators, and tumor size, histological grade, postoperative pathologic T staging of colorectal cancer (P values are "0.05).Conclusions:1.64-slice spiral CT three-dimensional imaging the colon can effectively compensate for the shortage of colonoscopy.2.reconstruction of colorectal tumor vascular volume density of blood vessels can be used as the level of preoperative prediction of colorectal cancer lymph node metastasis and distant metastasis in one of the important indicators.3.64-slice spiral CT three-dimensional imaging fast, safe, accurate, and colorectal cancer had a higher preoperative clinical value.
