Objective:To analyze the clinical characteristics, diagnosis, treatment and short-term prognosis of combined hepatocellular carcinoma and cholangiocarcinoma (cHCC-CC).Methods:Data from 17 patients who underwent hepatectomy and were pathologically confirmed as having cHCC-CC in The First Affiliated Hospital of Zhe Jiang University From April 2009 to February 2010 were collected. We retrospective analysis of its clinical pathological characteristics, diagnosis, treatment and prognosis in addition to indices including of sex, age, hepatitis B virus infection, cirrhosis, AFP, CA199, tumor size, disseminated nodules, thrombosis, invasion of adjacent, lymph metastasis, distant metastasis, TACE. The early overall survival rates were analyzed by Kaplan-Meier method.Results:None of the 17 cases of cHCC-CC was accurately diagnosed. The average age of the patients was 53 years (27-76 years). The percent of female patients was 64.7%. HBsAg positive was present in 70.6% and AFP expression (>25 ng/mL) was found in 68.8% of the patients. The cases with a tumor diameter less than 5 cm accounted for 38.9% and disseminated nodules were found in 44.4% of the total cases. Macroscopic tumor thrombosis was found in 22.2% and adjacent organ invasion was observed in 16.7% of the patients. The 6-,12-, and 18-months overall survival rates of the 17 cases that were followed up were 93.8%,86.5% and57.7%, respectively.Conclusion:cHCC-CC is difficult to diagnose preoperatively. The diagnosis depends on pathological examination. Surgical resection is the main treatment. The prognosis of cHCC-CC is poor, and Postoperative tumor recurrence rate is high.
