[Purpose] We investigated the effect of neoadjuvant chemoradiotherapy in treatment for advanced rectal caner.[Methods] A total of 29 advanced rectal cancer patients were first treated with preoperative chemoradiotherapy. Patients received radiotherapy five times a week at a dose 2Gy, daily up to 36-50Gy in 3-5 weeks. Concomitantly patients received continuous venous or oral chemotherapy with 5-Fu. Operations were performed 6-8 weeks after neoadjuvant therapy. Before the operation patients received preoperative chemotherapy with FOLFOX 4 regimen. Patients received adjuvant chemotherapy with FOLFOX 4 regimen. The follow-up were arranged every 3 months after surgery.[Results] Cancers shrank from a mean diameter of 6.10cm to 2.86cm,down size rate was 53.1%.17patients got down-staging,7 patients were found no cancer cells after surgery, reached complete remisson. Curative resection with sphincter saving operation were carried out in 9 of 24 patients, the rate of sphincter preservation was 38%.3 of 13 patients received Miles operation got infection of incisional wound. All side effects were mild and well tolerated.3 of them were found local recurrence,7 of them got metastasis,3 patients died, with a 3-year survival rate 89.21%.[Conclusion] Neoadjuvant therapy in advanced rectal cancer patients showed a efficacy in down-staging, in increase of resectability and sphincter preservation. The side effects were mild and well tolerated. Patients dealing with Miles operations suffered more risks in infection of incisional wound after noeadjuvant radiotherapy.
