Comparison Of Predictive Value Between Intrarenal Arterial Resistive Indeces, Pulsatility Indeces And Chronic Allograft Damage Index Early After Renal Transplantation

Objective To explore the relationship between intrarenal arterial resistive indeces (RIs), pusatility indeces (PIs) and the Chronic Allograft Damage Index (CADI) of protocol biopsy early after renal transplantation. Secondly, to compare the predicting value of long-time renal function and graft survival between these indeces.Methods We retrospectively analysed 132 renal transplant recipients,who underwent RI and PI assessment within 22±11 days after transplantation, and protocol biopsy within 39±13 days. The relationship between RIs, PIs and CADI was explored. During the 69±22 months after transplantation, the predicting value of long-time renal function and graft survival between these indeces was also compared.Results The intrarenal arterial RIs and PIs were significantly correlated, both RIs and PIs were significantly correlated with CADI.The renal arterial RI and PI were significantly correlated with the estimated Glomerular Infiltration Rate (eGFR) 0-6 years after transplatition, although CADI were not. After calculating the Relative Risk (RR), we found the renal arterial RI and PI were the strongest predictior of the prognosis. When the patients were subdivided to RI> 0.7 and PI> 1.4, we found the patients had lower RIs and PIs would be haven higher eGFR, they had more chances to occur acute rejection, protenuria (>1g/d), serum creatinine increase>50%, and graft lost (P<0.05).Conclusion Compared to CADI, renal arterial RI and PI has better predictive value for recipients of primary and secondary endpoints, such as acute rejection, protenuria (>1g/d), serum creatinine increase>50%, and graft lost.
