ObjectivesBy collecting the information of the Hepatitis B Cirrhosis Patients inward in Guangdong Provincial Hospital of TCM, to find out the TCM Syndrome and Treatment Regular Rules of Hepatitis B Cirrhosis, in order to confer reference for the TCM diagonose and treatment of the Hepatitis B Cirrhosis.MethodsBy using the method of clinical epidemiology and DME,593 cases according with the standard of the diagonosing Hepatitis B Cirrhosis from Guangdong Provincial Hospital of TCM were reviewed in a retrospective study from 2002 to 2010. The data were analysed by descriptive analysis, Frequency analysis with SPSS13.0 software package. So the TCM Syndrome and Treatment Regular Rules of Hepatitis B Cirrhosis were studyed deeply.Result1. The general conditionsThe 593 cases came from the hepatopathy dept and the Gastroenterology dept. By analysising the clinical data, we find that the proportion of male and female was 4:1(male 475, female 118). The age ranged from 18 to 92 years old and the average age was 52.94 years old. The average course of disease was 11.79 years. Every case was in hospital for 1.9 times averagely and the ratio of time after time in hospital was 35.8% and the mortality rate was 7.75%. Among the 593 Patients who suffered from Hepatitis B Cirrhosis,497 Patients are on the loss compensatory stage, most of them died of complications. The most common complications occurred in 165 cases, including Upper Gastrointestinal Hemorrhage (57 cases), Primary Liver Cancer (107 cases), Spontaneous Bacterial Peritonitis (31 cases), Hepatic Encephalopathy (27 cases), Hepatorenal Syndrome (27 cases).2. Syndrome CharacteristicAccording to the common symptoms, we can see that more than 20 kinds of symptoms occurred in the 593 cases. The clinical symptoms appeared as folows: fatigue, abdominal distention, both lower limbs edema, jaundice, dry mouth, bitter taste, hypochondriac pain, poor appetite, poor sleep, oliguresis, loose stools. By analysising statistically, fatigue and abdominal distention occurs most frequently, accounting for 85.2% and 54.8% respectively.In the aspect of tongue and pulse condition, the percentage of dark-red tongue was 46.4%,and 24.5%,8.8% in the yellow and greasy fur on the tongue and white and greasy fur. The wiry thready pulse, wiry slippery pulse are the main pulse, accounting for 49.1% and 20.9% respectively.In the aspect of TCM syndrome types, The pathogenetic condition of the Hepatitis B Cirrhosis was complicated and the TCM syndrome types are changeable. The common types are as follows:the Liver depression and Spleen deficiency, the Damp-heat Accumulation Type, the accumulation and stasis of Damp-heat, the damp-stasis accumulation, the retention of damp-turbid substance, the Yin deficiency of liver and kidney and the Yang deficiency of spleen and kidney etc.3. Treatment Regular RulesIn the aspect of treatment condition,the main Therapeutic Principle of TCM was taking a both temporary solution and a permanent cure. With the high frequeney of the medical functions the funetions of the medieine that appear more frequently are listed by the order from more to less:disbiting water and percolating dampness, Qi-benifiting and spleen-strengthening, moving qi, quickening the blood and transforming stasis, clearing heat, enriching the blood, strengthening aPPetite, supplementing liver and kdneys. The common used Chinese medicines are Poria, Atractylodes, Glycyrrhiza, Bupleurum, Artemisia capillaries, bitter orange, alismatis thizome, Polyporus, savia, Radix pseudostell, Curcuma, Magnolia, achyranthes root etc. Here we got the list of common used Chinese medicines.ConclusionBased on the anlysis of the clinical materials of the 593 cases, it can be known that the patients of the Hepatitis B Cirrhosis had features such as more male patients, older in age, longer in course of disease and more times of being inhospital, more complications and high mortality rate. The pathogenetic condition of the Hepatitis B Cirrhosis was complicated and the TCM syndrome types are changeable. The evil of the damp, heat and blood stasis are the main pathological factors and products. The pathologic stages showed the difference between the qi and blood system. Furthermore, the syndrome type occurs with compound types,and the Liver depression and Spleen deficiency is the most common type.For the complication of the pathogenetic condition, unified medicine therapy has been taken commonly. Soothing Liver and strengthening spleen, disbiting water and percolating dampness, clearing heat and transforming stasis are the common treatment methods. So, the differentiation of Hepatitis B Cirrhosis should adopt the method of combination of Disease and Syndrome Differentiation, and select drugs by distinguishing stages, and then combined other treatment methods.
