| The following two aspects are concerned in this dissertation to discuss the perimenopausal syndrome systematically in the field of tuina of traditional Chinese medicine.1. Literature researchOn reviewing the knowledge of etiology and pathology of perimenopausal syndrome in modem medical science, the author pointed out that estrogen were closely related to women's perimenopausal syndrome. The estrogen level lowered excessively, which led to the dysfunction of hypothalamic-pituitary-ovarian axis. As a result, it would further cause perimenopausal syndrome. As far as its treatment is concerned, at present, modem medical science adopts mainly the treatment according to symptoms and the estrogen substituting treatment. Although curative effect of estrogen substituting treatment is confirmed clinically, it causes adverse reactions. Therefore, there are different opinions on the estrogen substituting treatment and its safety.On reviewing the etiology and pathogenesis, treatment selection on syndrome differentiation of perimenopausal syndrome in TCM, the author pointed out that the history of the cognition of physiology and pathology for the premenopause, climacteric, postmenopause stage in TCM is very long. Acupuncture and tuina has a desirable effect on perimenopausal syndrome, especially, regulating its functions of the reproductive endocrine and vegetative nerve system.Pointing Therapy is one of the major schools of tuina. Based on the theory of Yin-yang, Qi-blood, Zang-fu viscera and Channels-collaterals, Pointing Therapy has a desirable effect on perimenopausal syndrome in clinic. In addition, Lin Chaoxiong has a great experience on research of Pointing Therapy. On reviewing the cognition of perimenopausal syndrome in both Traditional Chinese Medicine and modem medical science, the author pointed out that there is stable and forceful theoretic foundation and significant clinical meaning to probing into the effect of Pointing Therapy for the treatment of perimenopausal syndrome and its reproductive endocrine mechanism.2. Clinical StudyObjective:In this paper, using Pointing Therapy and Livial to treat perimenopausal syndrome have been observed the curative effect and done comparative analysis.Methods:60 cases of perimenopausal syndrome sufferers were randomly assigned to treatment group (n=30) and control group (n=30). Treatment group was treated by Pointing Therapy twice a week,3 months in all, while control group was dealt with Livial 2.5mg once per day,3 months in all. Three months made up an observing therapeutic course, measured each index of both groups before and after treatment respectively. The indexes were as follows:(1) The improved method of Kupperman grade point was adopted to estimate symptoms and curative effect.(2) Serum Estradiol (E2), Follicle-stimulating Hormone (FSH), Luteinizing Hormone (LH) levels, and relational indexes of liver and kidney were observed before and after treatment respectively.Results:(1) After treatment, the total effective rate of treatment group was 96.67ï¼…, while markedly effective rate was 83.33ï¼…; the total effective rate of the control group was 93.33ï¼…, while markedly effective rate was 63.33ï¼…. The main symptoms of both groups were improved greatly and Kupperman grades decreased significantly after treatment (P<0.05), and curative effect of the treatment group was better than control group.(2) There was no significant difference between treatment group and control group in the level of serum E2,FSH,LH before treatment (P>0.05). After treatment, the level of serum E2 of both groups were increased significantly (P<0.01); the levels of serum FSH and LH were significantly decreased (P <0.01). But there was no significant difference in serum E2, FSH and LH level between two groups (P>0.05).(3) The investigation of adverse reaction in recentness:ecchymosis occurred in 2 cases of treatment group(6.67ï¼…); there were 2 cases (6.67ï¼…) suffering from headache,4 cases(13.33%) suffered from gaining weight and 3 cases(10%) suffered from colporrhagia during medication. The function of the liver and kidney of both groups were normal before and after treatment. The occurrence rate of adverse reaction in the control group was higher than that of the treatment group (P<0.01).Conclusion:(1) Pointing Therapy has an obvious effect on the treatment of perimenopausal syndrome, while the total effective rate up to 96.67%. Pointing Therapy not only could improve various kinds of clinical symptoms notably, but also hasn't obvious poisonous side effect.(2) Pointing Therapy could regulate the hypothalamic-pituitary-ovarian axis, and could improve the endocrine environment by raising serum E2 level and reducing the serum FSH, LH level of the perimenopausal woman.(3) The curative effect of Pointing Therapy on perimenopausal syndrome was better than the control group, and the safety of Pointing Therapy is better and the cases occurrence rate of untoward reactions is lower than that of the control group. |
