The Clinical Study For Treatment Of Intraarticular Fractures Of Distal Radius With Three Methods

Objectives:By three methods of the intra-articular distal radial fra-ctures.To choice.the intraarticular fractures method of treatment.Methods:Uses the retrospective analysis method.Analyzes in May, 2007～2009 year.Our orthopedic hospital treated 96 case of closed distal radius fractures in patients with clinical data or communication through the outpatient follow-up 6 months to 8 months, an average of 12 months.The age of (30-80)years old.91 patients (93 sides) were eventually re-.the manual reduction and plaster immobilization 36 examples (36 sides).External fixation 24 cases(26 sides),Open reduction 31 cases(31 sides).Carrying on through the outpatient service or the communication way make a follow-up visit 6 months to 18 months,The average makes a follow-up visit for 12 months, From the bone fracture cicatrization time,the wrist function resume and aspects and so on phantom study curative effect analysis carry on the analysis comparison.Result:All patients were healed. In fracture healing time,the manual reduction and plaster immobilization group (4±0.7 week) significantly better than the other two groups (external fixation group was 5±0.6 weeks,Open reduction and internal fixation group, the longest healing time 7±0.2 week);After treatment of distal radius anatomy recovery,External fixator group (good rate 88.46%) and the Open reduction group(high rate of 90.32%) no significant difference between,But they are better than the manual reduction and plaster immobilization group (high rate of 63.98%); Functional rehabilitation in the wrist,the External fixator group(good rate 84.62%) and the Open reduction group (good rate 87.10%) no significant difference.The both functional effect is superior to the manual reduction and plaster immobilization group(high rate of 55.56%);the Cases of complications in this group is mainly pain and joint stiffness,especially the manual reduction and plaster immobilization group is the highest incid-ence.In the satisfaction,three cases of patient is no significant differrence.Conclusion:For intra-articular distal radius fractures, especially dis-placed and unstable fractures obviously should be encouraged to choose external fixation or Open reduction, the manual reduction and plaster im-mobilization is suitable for fractures with no displacement.Researching in this group can be seen,there is no absolutely advantage in a treatment method can handle with all of the intra-articular distal radius fractures.In the choice of treatment method, we not only determine the type and severity of fracture,but also combine with patient's physical condition, in economic conditions or other factors,individualized treatment plan,choosing the best method of treatment or more of the combined treatment,and striving to be so treated with the efficacy of patient satisfaction.
