Objective: To subject the scope of the breast cancer surgical resection by Histopathology and molecular biology and to exposure Livin and Fhit breast conserving surgery if as a cutting edge of molecular feasibility of surgery for breast-conserving surgery radiotherapy target volume definition of reasonable security provide a theoretical basis.Methods: 30 cases of modified radical mastectomy of breast cancer specimens in our hospital were collected under sterile conditions ,fresh breast cancer tissues and adjacent tissues, the amount of steel ruler to take body samples from the adjacent pitch, adjacent to take the nipple and tumor centers and vertical connections Line 4 direction of 1cm, 2cm, 3cm a total of 13 parts of fresh tissue, total about 4g, to be part of the 10% formalin-fixed, for the production of HE staining and immunohistochemistry and the other part to be -80℃refrigerator after cryopreservation carried out after extraction of RNA using RT-PCR to detect the expression of Livin and Fhit in 30 cases of breast cancer and adjacent tissues from 1cm, 2cm, 3cm.Results:1,Livin protein expressed in cancer and adjacent tissues 1cm, 2cm, 3cm followed by 66.67%, 20.83%, 2.50%, 0%, LivinmRNA in cancer and adjacent tissues 1cm, 2cm, 3cm expression was followed by 73.3%, 19.17%, 3.33%, 0%. Fhit protein expressed in carcinoma and adjacent tissues 1cm, 2cm, 3cm followed by 46.67%, 76.67%, 82.50%, 86.67%, the expression rate of FhitmRNA in cancer and adjacent tissues 1cm, 2cm,3cm followed by53.33% ,73.30%, 85.00%and 87.50%.2,Livin was high expressed and the Fhit was loss expressed in Breast cancer, There was no correlation between in Livin expression and patient age, tumor size, histological grade, estrogen receptor (ER), but with the clinical stage and lymph node metastasis; There was also no correlation between in Fhit-positive Expression and the patient's age, estrogen receptor (ER), clinical stage,but with tumor size, histological grade, lymph node metastasis.3,There was only one case of tumor cells at 1cm residual in 30 cases of breast cancer, far, near the nipple end and its vertical line of direction may be related to its histopathological type and tumor size.4,Expression of breast cancer Fhit and Livin expression was negatively correlated (r = -0.544 P <0.01).Conclusion:1,Livin was high expressed and the Fhit was loss expressed in Breast cancer, Livin possibly through PI3K/Akt way down Fhit, may be one of the ways of promoting breast cancer invasion and metastasis.2,Livin and Fhit in breast cancer and adjacent 1cm, 2cm, 3cm at the trend of expression was different, Livin, and Fhit could serve as a cutting edge study of breast cancer molecular marker, 2cm or less can be seen as field cancerization.3,Molecular biology is superior to histopathology in detecting the ablity of invasing in breast cancer, the joint use of both methods can improve detection sensitivity.4,1cm away from the adjacent range is a more reasonable intraoperative electron irradiation area.
