New Rural Cooperative Medical Care Research On Out-patient Co-ordination In Taiyuan

Objective:On the trend of the new rural cooperative outpatien compensation is from the family account mode to the co-ordination mode conversion, we combined with the actual funding levels amd the outpatient consumption of farmers in Taiyuan to explore whether is feasible to implement the patient co-ordinate program in the current socio-economic development; At last, we proposed the best implementation system of outpatient co-ordination in Taiyuan.Methods:According to the consumption levels of farmers in the 10 Counties/Districts of Taiyuan, we used the "balance factor method" to measure the best overall compensation ratio. If the compensation ratio was in the range of 20%-80%, it meant our program would be feasible. Our data was from the random samples of 7931 village families and random outpatient consumption prescription samples. We used the percentile method and system method to determine the other parameters of the patient co-ordinating program.Results:The two-week prevalence rate of the rural residents was 4.75%, chronic disease prevalence was 6.03%, lower than the national health services survey, that was 17.7%and 17.1%; the average two weeks attendance rate was 1.5%, lower than the rate of 15.2% in the fourth national health services survey.46.99% patients did not have the attendance. Average Village-level single prescriptio price is 27 yuan, the township-level is 31 yuan, per capita outpatient consumption is 28.13 yuan. For the more conservative out-patient co-ordinate programs, we need to make use of the national two weeks attendance rate to estimate the compensation rate.Based on the quantile 75%, the stopped line of outpatient co-ordination system was 30 yuan, from 0 to 30 per pay line, the compensation ratio was fluctuating between 25-41%. That means the outpatient co-ordination system will be feasible in Taiyuan.Considering the consumer characteristics and the the best compensation ratio in different counties and the other policy parameters, we worked out the outpatient co-ordination program consistent with the actual situation in Taiyuan at last.Conclusion:The more conservative strategy to measure the overall program in Taiyuan area still had the viability, then the other districts could explore the appropriate out-patient program, the outpatient co-ordination replace the family account was feasible in the rural areas.
