| Anatomic Studies on sulcus of vertebral artery of atlas and vertebral pedicle screwingObjective:To take statistics of the incidence rate of anatomical variations in the sulcus of vertebral artery of atlas in Chinese people; to make anatomical measurements and statistical analysis on the anatomically-variated sulci of vertebral artery; therefore to identify whether the fixation with cervicle pedicle screws could be applied.Methods:160 cases of Chinese drying atlas specimens were chosen to take statistics of the incidence rate of variations in the sulcus ring and make specific classifications. Domestic vernier calipers were used to make anatomical measurements on variated specimens, for deciding whether the fixation with cervicle pedicle screws were able to be employed in the cases.Results:Among the 160 cases,26 cases of variations existed in the sulcus ring,9 cases of whole-ring variations and 20 cases of half-ring variations. The minimum heights (H4) of the screw, within bones, in the sulcus of vertebral artery, were respectively 2.95±0.22mm at the side of the rear-ring variations,2.94±0.32mm at the side of the lateral ring variations,4.72±1.04mm at the side of the upper axon variations, 3.18±0.40mm at the side of the lower axon variations,4.09±0.72mm at the side of the rostrum variations.Conclusion:According to the research results, the conclusion is as follows:to 350 mm diameter pedicle screws for standard):1 The variations in the sulcus of vertebral artery of atlas have an obvious influence on the vertebral pedicle screwing;2 It is not suitable for the whole-ring variations and the lower axon variations in the ulcus of vertebral artery of atlas to fix screws in the cervicle pedicle;3 the rostrum variations in the ulcus suit the screw-driving method of Tan Mingsheng to decide the screw-driving spots, while the rostrum variations and the upper axon variations in the ulcus should be designed individual for the the screw-driving spots. Because of this experiment research objects are atlas dry osseous specimens, in the clinical application of radiographic results and practical measurement bony anatomical there might be error between the author suggest, in clinical operation, vertebral artery sulcus ring and the variation not under axon suggested pedicle screws, rise or kiss variation, recommend preoperative ashdod did CT scan,3d reconstruction according to results or individual nail way design and navigation positioning, medical reduce complications. |
