There is a protection mechanism, on the surface of normal bladder mucosa, that can prevent inflammation, bacterial clumps, carcinogenic substances etc from adhering to the bladder mucosa above. For some reason, when this protection mechanism is destroyed, all kinds of inflammatory substances in the urine would worsen the damage of bladder mucosa, causing a series of bladder inflammation. Some scholars have put forward the bladder barrier theory that the tight junction between bladder epithelial cells, specialized cells of the umbrella, contributing to bladder mucosa block up the invasion of bacteria, certain molecules and ions. This barrier is composed of the Glycosaminoglycan (GAG) , which is distributed the surface of the bladder lamina propria layer. Some scholars have conceived to instill the Hyaluronic acid (HA) [1] , heparin[2] and multi-pentosan[3] in the bladder for Interstitial cystitis (IC) patients, to repair damaged GAG layer, recover protective barrier of bladder mucosa, efficacy significantly.Objective:We investigated the effects of the various molecular weight and concentration of sodium hyaluronate (SH) on the bladder epithelium after mucosal injury healing.Methods:Rats were randomly divided into A, B, C, D, E, F, totally, of 6 groups. A group was used for the blank control group, B for the negative control group, C for the low concentration of small molecular weight group, D for small molecular weight of high concentration group, E for the large molecular weight of low concentration group, F for the high concentration of large molecular weight group of eight each. Injury in rats was created by hydrochloric acid applied on bladder mucosa. And then, bladders were perfused with large and small (1 650 ku, 1 100 ku) molecular weight, high and low (0.2%, 0.08%) concentrations of SH solution. With conventional staining techniques, we observed the pathological bladder mucosal injury and inflammatory infiltration, and with the technique of immunohistochemistry, SH of heat shock protein 60, 70 (HSP60, HSP70) expressing in the mucosa affected.Results:Endoscopic pathological changes,C, D, E group had mucosal hyperemia, a small amount of bleeding, F group, scattered in small punctate hemorrhage.Concerning the results of HSP60 expression, F group, when compared with B, C groups, There was a significant with them (P <0.01), and when compared with D, C groups it was a significant (P <0.05), and when compared with F, E groups it showed no significant(P>0.05). Concerning HSP70 expression, F group, compared with the B, C, D, E group, showed a significant (P <0.01), and compared with D, C groups it showed a significant (P <0.05).Conclusion:Increasing molecular weight and concentration of SH evidently improved the repair capability of bladder mucosa after injury, which might be related to an augment of the viscosity of the SH.
