OBJECTIVE:To investigate the feasibility and advantage of children's tonsillectomy with monopole electronicknife with electric coagulation compared with peel off method tonsillectomy.METHOD:Thirty patients treated with tonsillectomy of both sides age from 3 to 14 were included. It was decided randomly the coagulation side. The operating time, blood loss during operation, the wound recover, the degree of pain, diet and complication were observed and recorded detailedly. And then analyse these above indexes.RESULT:The average dissection time of surgery was 6.5±2.6 min for electrocoagulation tonsillectomy compared with 5.6±2.7 min for peel off method tonsillectomy. The difference of dissection time between the two groups is considered to be not statistically significant(P=0.14).In the electrocoagulation group, haemostasis time, operative time and intraoperative blood loss were respectively 2.5±2.8min,9.0±4.3min and 6.0±6.8ml;while as in the peel off group, haemostasis time, operative time and intraoperative blood loss were respectively7.7±3.9min,13.2±5.6min and 18.9±8.8ml.The indexes mentioned above have the statistical difference (haemostasis time P=0.00, operative time P=0.00, intraoperative blood loss P=0.00). The electrocoagulation tonsillectomy shorten the operation time and reduce the intraoperative blood loss compared with peel off method tonsillectomy. The deference in fascicle exposure score between the two method side was statistical significant with statistic index Z=-4.916, P=0.00. The electrocoagulation tonsillectomy is associated with lower fascicle exposure score. The postoperative pain and recover of tonsillar fossae in the electrocoagulation was similar to that in the peel off tonsillectomy.CONCLUSION:The application of electrocoagulation tonsillectomy in children was safe, efficient and feasible. The electrocoagulation tonsillectomy has established over peel off tonsillectomy in shorting operation time and in reducing intraoperative blood loss. Besides, the electric coagulation tonsillectomy not only has a clear-cut view but also minimizes damage to adjacent tissue. Postoperative morbidity of electrocoagulation was similar to that in the peel off tonsillectomy.
