| Objective:To study the effect of removing lipopolysaccharides(LPS) in blood and the influence on prognosis by direct hemoperfusion(HP) with polyvinylidene difluoride-serine(PVDF-Ser) in pigs with sepsis,and the non-specific adsorption of other components in blood with PVDF-Ser column, in vitro.Methods:(1)The removing effectiveness on LPS of 3 ligands of different amino acids(serine,arginine and asparagine)was examined in dynamic adsorbtion test in vitro and the best one was chosen.(2)Sixteen male family pigs were injected endotoxin(1ug·kg-1·h-1 intravenously) for 2hs and randomly assigned to either group(n=8 per group):only endotoxin intravenously without HP(control group);HP with PVDF-Ser volumn for 2hs after endotoxin (experiment group).Blood samples were drawn at baseline and 1,2,3,4,5h after endotoxin injection for the measurement of endotoxin and plasma cytokine concentrations.The mortality rates at 72h after endotoxin administration and histopathology of the lung,intestine and liver in both groups were observed and compared.(3)The biochemical components in pig blood were examined before and after circulating adsorption with PVDF-Ser column for lh,in vitro.Results:(1)The dynamic adsorption qulity of PVDF-Ser column on LPS was 51.572EU/g,in vitro and was obviously better than those of the other amino acid ligands.(2)The concentration of endotoxin in experiment group was significantly lower than in the control group(0.031±0.016EU·ml-1 vs 0.141±0.027EU-ml-1,p=0.009) and the concentrations of tumor necrosis factor-α(TNF-a), interleukin-6(IL-6)in experiment group were also lower than in the other group(p< 0.05).The histopathology examination of the lung,intestine and liver showed tissue damage in the control group was more severe than in the experiment group.The mortality rates at 72h after endotoxin injection were 25% and 87.5% for the experiment group and control group,respectively(p=0.003).(3)The results showed that circulating adsorption with PVDF-Ser column made a 13.3±2.4% reduction in the level of Chlorine ion in pig serum(p=0.015),and no significant influence on other blood components were found.Conclusions:Direct hemoperfusion with PVDF-Ser column effectively adsorbed endotoxin in vivo,had an inhibitory effect on plasma cytokines and reduced the inflammatory responses and mortality in endotoxemia. |
