Objective:To study the influence of the reconstructions of the pancreas residual on the physiological functions of pancreas and stomach, and evaluate the physiology, security and clinical applications of Binding Pancreaticogastrostomy..Methods:In this prospective clinical study,21 cases were recruited from general surgery department of SirRunRunShaw hospital between May 2009 and January 2010. Of these cases,13 patients underwent Binding Pancreaticogasrostomy (BPG) and 8 patients underwent BPJ after their pancreatic operation. To comprehend the physiological effect of these two anastomosis, some important biochemical parameters of blood, gastric liquid and abdominal led, which reflect the physiological functions of stomach and pancreas, were determined pre- and post-operation, respectively. Results:The two treatment groups were comparable in multiple parameters, including demographics, medical history, preoperative laboratory values, and intraoperative factors, such as operative time, blood transfusions, pancreatic texture etc; while, the anastomosis time of BPG group was significant shorter than that of BPJ group (34.3±5.4 vs.52.1±14.5 minutes) (p< 0.001); The related biochemical parameters of blood, gastric liquid and abdominal led pre- and post-operation, which reflect the physiological functions of stomach and pancreas didn't exit significant difference between the two groups, and had the similar tends in change; Moreover, there were not significant difference in postoperative anus exhaust time, the time of taking off gastric tube, feeding time, weight change and discharge time of the two groups. Importantly, postoperative complications in BPG group is less and light when compared to with BPJ。Conclusion:BPG is a safe and physiological anastomosis that can significantly reduce pancreatic fistulation and other fewer complications. It is also a simple operation with short anastomosis time and truly no suture. Due to these, BPG could be highly recommended in clinical application.
