The Occurrence Of Post-stroke Depression And Venlafaxine Clinical Study Of Early Intervention

Purpose:Post-stroke depression (post-stroke-depression,PSD)is a common complication of stroke patients,and its performance appears to emotional or mood disorder based on depression,decreased interest,sleep disturbance, It is secondary depression。It not only bring patients physical and mental pains,but delay neurologic impairment recovery time,increase the damage of cognitive function,and severely affected quality of life of patients.Venlafaxine is an precise effective and new structure of phenylethylamine antidepressant,is 5-serotonin and norepinephrine dual reuptake inhibitors.The mechanism is characterized by dual mechanism,to limit the re-uptake sites of 5-serotonin and norepinephrine in the synaptic, also can inhibit the dopamine reuptake slightly, affecting the spirit and behavior, improve depression patients symptoms.The dose of flexibility enable improved reactivity with the increasing dose.Because of the lack of receptor affinity, there is no Previous antidepressant side effects,thereby enhancing the drug safety and tolerance.This study on follow up of acute stroke patients,screening patients with post-stroke depression,and given the patients venlafaxine antidepressant treatment,to explore affect of venlafaxine on depression in the patients,recovery of neurological deficits and daily living skills.Method:Select 110 cases of acute stroke patients from July 2008 to November 2009 in the hospital.Assess HAMD in the onset of two weeks. HAMD score> 7 points and clinical depression symptoms as a diagnostic criteria for depression after stroke,statistics the incidence.The patients diagnosised of depression after stroke were randomly assigned to venlafaxine in the treatment group and comparison group,.The conventional control group was given routine treatment of Neurology, and venlafaxine in the treatment group was given venlafaxine oral 22 weeks after two weeks of the onset.Assessed patients with the Hamilton Depression Rating Scale (HAMD),activities of daily living scale (ADL),National Institutes of Health Stroke Scale (NIHSS)in the course of 10 weeks and 22 weeks,and observed depression changes in symptoms and side effects and safety of venlafaxine.Result:①A total of 48 cases of disease in patients with depression in 2 weeks,the incidence of depression after stroke was 43.64%.A total of 61 patients with depression in 12 weeks,the incidence of depression after stroke was 55.45%.a total of 64 patients had depression in 24 weeks,the incidence of depression after stroke 58.18%.Mostof patients are mild or moderate depression.②HAMD score of the two groups before treatment was no significant difference(P>0.05).Venlafaxine treatment group compared with pretreatment, HAMD scores decreased significantly (P <0.05). Conventional treatment group after treatment compared with pretreatment HAMD scores increased significantly(P<0.05).The HAMD score of Venlafaxine treatment group is lower than that of the conventional control group.There is significant difference(P<0.05).③NIHSS score before treatment of two groups were not significantly different(P> 0.05), The NIHSS score of Venlafaxine treatment group is lower than that of the conventional control group.There is significant difference(P<0.05).④ADL score before treatment of two groups were not significantly different (P>0.05),The ADL score of Venlafaxine treatment group is lower than that of the conventional control group.There is significant difference(P<0.05).Conclusion:①The incidence of depression after stroke is high.2 weeks after stroke onset depression rate was 43.64%.the incidence of depression after 12 weeks was 55.45%,andafter 24 weeks,the incidence of stroke was 58.18%.Most of them are mild or moderate depression.②Early Venlafaxine treatment is practical effective in improving clinical symptoms in patients in post-stroke depression.③Early Practical venlafaxine may Promote the recovery of neurological function and improve the quality of life in patients with post-stroke depression to some extent.④.Venlafaxine is Side effects,safe and effective drug treatment of depression after stroke.
