| Objective To examine the whole body MRI on multiple myeloma invasion and the clinical values of the whole-body MRI in multiple myeloma classification.Materials and Methods 30 cases in March 2008 to December 2009 in Shanxi medical second school of clinical medicine of untreated patients with MM, it is determined that no contraindications of MRI when row axis systemic bone MRI and MRI, the series includes T1WI and STIR. At the same time collecting records individual clinical laboratory in patients with serum markers. The patients body MRI with the support of Durie and Salmon PLUS clinical classification, bone MRI in the axis with the support of Durie and Salmon clinical classification, and according to the clinical laboratory indexes do clinical classification of tumor burden.Results 30 cases of patients, whole-body MRI with the support of Durie and Salmon PLUS clinical types are as follows:stageâ… have five cases, stageâ…¡11, stageâ…¢14, there were 471 Department damage; bone MRI in the axis with the support of Durie and Salmon clinical types: stageâ… have 11 cases, stageâ…¡14 cases, stageâ…¢five cases, there are 249 damage; clinical classification of tumor load:stageâ… have 8 cases, stageâ…¡12 cases,10 patients with stageâ…¢. For the detection rate, the whole body MRI was statistically superior to MRI imaging of the axial skeleton (p< 0.001, Wilcoxon's signed rank test,n=30,T=15.5,p< 0.05,mind=465,a=0.05). Seventeen patientswere understaged with the axis MRI compared with the whole body MRI, which was statistically significant (p< 0.005, chi-square test).The whole-body MRI with the support of Durie and Salmon PLUS clinical and MRI in the axis of the bone under the support of Durie and Salmon PLUS clinical type comparison, the former and tumor load classification with relevance (consistency test, p<0.05,kappa 0.599>0.446). MRI images displayed in the study of bone marrow disease distributed as follows:will only exist in the axis of the bone marrow lesions, with only the central axis of the lesions in bone marrow and two parts have bone marrow diseases. Simple line axis bone MRI more systemic MRI examination will result in some cases undetected. Purely on the central axis of the vertebral bone compression study found; in the axis of vertebral fracture of bone and bone marrow lesions infringement of cortical bone and physiological stress have a certain relevance, this forecast MM in fracture risk have important significance.Conclusions Whole-body MRI leads to a significantly higher detection rate and staging in patients with multiple myeloma.The violating of cortical bone and the compression on vertebral body may be one of reasons to the vertebral body fracture in MM. |
