ObjectiveTo investigate the expression of phosphorylation signal transduction and activators of transcription-3 (p-STAT3),vascular endothehal growth factor (VEGF) and Matrix metalloproteinase-2 (MMP-2) in middle ear cholesteatoma epithelium of human.Then to analyze their corelations and roles in the hyperplasia and ossein destructiveness of human middle ear cholesteatoma epithelium.Methods39 cases of cholesteatoma specimens were collected at time of surgery and fixed immediately.According to the extent of ossein destructiveness,39 cases were divided into four groups:Ⅰdegree (8cases);ⅡDegree (13cases);Ⅲdegree (11cases);Ⅳdegree (7cases).At the same time,12 cases of the normal skin of external acoustic meatus were used as the control group.Decalcification was performed and the expression of p-STAT3, VEGF and MMP-2 were examined by using the technique of immunohistochemistry in all the cases.The images of the stained section in each group were analyzed.Results1.The positive imunostaining of p-STAT3 could be observed in the nucleus of endothelial cells in cholesteatoma,appeared as brown granular.And weak staining could be found between vascular endothelial cell,while there was no immunostaining for p-STAT3 in the normal skin of external acoustic meatus.The level of P-STAT3 in middle ear cholesteatoma was130.80±18.90,the level of P-STAT3 in the skin of external acoustic meatus was 76.59±13.72,there was statistical significance between them(t=10.917,p<0.05).2.Strong immunorespectivity could be observed for VEGF in the basal keratinocytes and over keratinocytess of cholesteatoma,localized in the cytoplasm. The immunostaining was stronger in cholesteatoma stromal cells and endothelial cells,while weak immunostaining in the basal layer cells of skin of external acoustic meatus and cholesteatoma stromal cells.The level of VEGF in middle ear cholesteato-ma was 126.97±13.37,the level of VEGF in the skin of external acoustic meatus was 94.80±18.05,and there was statistical significance between them (t=6.698,p<0.05).3.Strong immunorespectivity could be observed for MMP-2 in stratum granulos-um,stratum spinosum and basal layer of cholesteatoma,localized in the cytoplasm. The immustaining for MMP-2 was gradually stronger from stratum granulosum to basal layer layer.Weak immustaining could be observed in the basal layer cells and stratum spinosum of skin of external acoustic meatus.The level of MMP-2 in middle ear cholesteatoma was 126.97±13.37,the level of MMP-2 in the skin of external acoustic meatus was 94.80±18.05,and there was statistical significance between the them(t=6.698,p<0.05).4.The immustaining for p-STAT3,VEGF and MMP-2 were stronger as well as the degree of ossein destructiveness was stronger in human middle ear cholesteatoma epithelium.There was statistical significance between the groups(p<0.05)5.There was statistical significance between the level of p-STAT3 and VEGF in middle ear cholesteatoma (r=0.445,p<0.05),p-STAT3 and MMP-2 (r= 0.351,p<0.05);VEGF and MMP-2 (r=0.568,p<0.05)Conclusion1.The proteins of p-STAT3,VEGF and MMP-2 were highly expressed in human middle ear cholesteatoma epithelium. Maybe they were important in the proliferation of cholesteatoma.2.The p-STAT3,VEGF and MMP-2 may play an important role for the erosion behavior of the ossein in middle ear cholesteatoma.3.The p-STAT3,VEGF and MMP-2 had positive correlations in the proliferation and erosion behavior of cholesteatoma epithelium.
