Objective:To investigate the value and clinical effects of reverse distal femoral less invasive stabilization system(LISS)for the treatment of intertrochanteric comminuted fracture in the elderly.Methods:From December 2006 to November 2008,21 intertro-chanteric comminuted fracture in the elderly patients treated with reverse distal femoral LISS in Shandong University of TCM First Affiliated Hospital Orthopaedics were retrospectively analysed.There were 6 cases male and 15 cases female aging from 61 to 88,The average 77.8-year-old.13 cases were at left leg and 8 were at right leg.We observe the incision length,operative time,blood loss,and fracture healing time,postoperative complications and Harris hip score.To compare with the same period using various other extramedullary fixation treatment and were followed up 55 cases intertrochanteric comminuted fracture in the elderly.Results:There were the follow-up 13 to 19 months,an average of 15.5 months.No patients died during follow-up.In the LISS treatment group, lasted for no case of infection, internal fixation failure,and nonunion complications.The fracture healing time 8 to 13 weeks,with an average of 10.95 weeks,and the fracture healing rate was 100%.The functional evaluation was according to Harris hip score,which was 83 to 97 points,and the average was 91.43 points.There were a significantly different and statistically significant the LISS treatment group between the DHS group and the DCS group in the incision length,operative time,blood loss.Conclusion:The application of reverse distal femur LISS treatment of intertrochanteric comminuted fractures in the elderly effectively reduce the traditional extramedullary eccentric fixation loosening, which dominated strong enough to result in fixation failure and other complications from occurring and have a fixed solid,high rate of fracture healing,short operation time,surgery trauma,high security and so on.The application of reverse LISS increased the therapeutic effects of intertrochanteric fractures,and provided a new therapeutic approach worthy of clinical application and further study.
