Objective To explore the possibility and clinical effect of autologous mini-skin graft covered with acellular xenogenic dermal in treatment of large area skin defects.Methods 24 cases of extensive deep burn and 6 cases of large area skin avulsion are selected in my department from November 2006 to December 2010. When condition is stable,they are given escharectomy (tangential excision) or excision of skin avulsion,and all are transplanted the mini-skin graft ,which then covered with acellular xenogenic dermal. Postoperative observation: the rejection of acellular xenogenic dermal;wound exudation; time for wound healing;follow-up from 3 months to 2 years after healing to observe the appearance and flexibility .Results After 5 days of the operation, wound exudation obviously decreased; after 21 days the acellular xenogenic dermal gradually deactivated and exfoliated in dry scabs. There is no redness or rashes surrounding the wounds. The healing time for the large area deep burn was from 16 to 40 (23.04±6.56)days and large area avulsion skin injuries was from 18 to 35 (24.67 ±5.92)days. Follow-up for15 cases from 3 to 6 months after healing showed good epithelialization and pigmentation.There were superficial scars which showed slightly stiff and mild contracture. Follow-up for 8 cases from 1 to 2 years showed no obvious scars proliferation, little pigmentation,better recovery of flexibility and functonality and no contracture deformity.Conclusion Acellular xenogeneic dermal as a covering in treating large area skin defects by mini-skin graft can effectively reduce wound exudation , help to facilitate healing, alleviate the patients'pain and reduce scar formation.It is worth of clinical promotion and application.
