An Analysis On The Building Of The Medical Safety Alert System In Hospital

Objective: Make contrasts of the indication between patients involved in medical disputes and that of the ordinary patients according to an analysis on the medical safety affairs during the past five years of a top first-class hospital in central plains. Search for the signals of medical safety alert, build the grading of medical safety alert and provide basis for the building of medical safety alert system according to the clinical manifestation and results of laboratory examination.Methods: Analyse the situation of the medical disputes in the past five years according to descriptive statistics. Extract the medical records of the medical disputes in the past five years and extract an ordinary medical record next to it. Make frequency analysis , Chi-square test,t test and Logistic regression analysis by spss11.5 software.Results: Among the 144 medical disputes cases happened during 2005 and 2009, 96.5% were handled with negotiation, 3.5% were meditated by the courthouse,71.5% had no reason recorded, 80.5% had no handling process recorded. There is a striking difference between the patients involved in disputes and the ordinary patients with regard to whether they are diagnosed that very day and the kind of disease they got(P=0.036,P=0.029).63.2% of the patients involved in the disputes take an operation while 29.9% of the ordinary patients take an operation(P<0.05). The mortality rate of the patients involved in the disputes is 40.2% while that of the ordinary patients is 5.7%. 19.5% of the patients involved in the dispute change medical offices while 8.0% of the ordinary patients change medical offices(P<0.05). The distribution of the group consultation frequency is different between the patients involved in the dispute and the ordinary patients(P=0.000). The length of time of hospitalization is different strikingly between the patients involved in the dispute and the ordinary patients(P=0.013).There are four factors contributing to the occurrence of medical disputes happened to the patients hospitalized: death(P=0.000), operation(P=0.000), group consultation(P=0.044) and the variety of illness(P=0.017). It is 10.494 times more likely for the dead patients than the alive patients to occur medical disputes. It is 6.091 times more likely for the operated patients than the non-operated patients to occur disputes. It is 2.185 times more likely for the patients diagnosed by the group consultaion than the patients not diagnosed by the group consultaion to occur disputes. It is 2.752 times more likely for the patients suffered from various illness than patients suffered from one single illness to occur disputes. The distribution of the hemoglobin examination results and the blood sugar examination results are different between the patients involved in the dispute and the ordinary patients(P=0.023,P=0.021). It is 0.407 times more likely for the patients with a normal glucoseconcentration than patients with an abnormal glucose concentration to occur disputes. Conclusion: The situation of medical safety is severe and it is imperative to build the medical safety alert system. An clear and feasible alert mode is the basis of the operation of medical safety alert system. We should build the alert mode on account of the emergency patients, clinic patients and hospitalized patients. Search for the signals of medical safety alert according to the results , find different alert signals of different alert modes and divide every alert mode into three grades. Take the management of medical safety alert in full operation, make every alert index of clinic medical service more standard and practicable. In addition, handle the danger signal and guard signal respectively. Assess the emergency patients' condition and handle the situation according to the corresponding alert grade. Hospitalized patients should execute the inform system stricty when they are admitted in hospital. Intensify the effective communication between hospital and patients. Intensify the medical safety alert of the hospitalized patients according to the time when the patients enter the hospital, the situation of the change of medical offices, whether they have operation or not, the prognosis of disease and examination results of their illness. Strengthen the management of medical safety and optimize the handling mode of medical disputes. Improve the medical safety alert system to make the index of medical safety alert more concise and professional. Generalize the coverage area of medical safety alert system to realize the overall and harmonious development of the buliding of medical safety alert system.
