The Relation Between Peripheral Atherosclerosis And Coronary Heart Disease

Objective:To investigate the relation between peripheral atherosclerosis and coronary heart disease (CHD).Methods:Atherosclerosis of carotid,femoral was investigated with ultrasound in 74 cases who were skepticed coronary heart disease undergone coronary arteriography (CAG.). Among them 57 cases are CHD and 17 cases are normal. Stepwise multiple regression analysis and Logistical regression analysis were carried between the results of extracoranary artery ultrasound examination (include IMT, atherosclerosis plaque, cardiac risk factor) and CAG. In this system,the cumulative impaired seriousness of geometry, the accumulation of multi-place stenosis,the significance of stenosis location and the influence of lateral branch circulation,the dimension of distal blood vessel, the importance of the blood quantity and the myocardial function are all considerated. We divided stenosis into six grade:below 25%,26%～50%,51%～75%,76%～90%,91%～99% and 100%. We defined 0～25% 1 score; 26%～50%,2 score; 51%～75%; 4 score; 76%～90%,8 score; 91%～99%,16 score; 100%,32 score. The coronary artery is divided into 15 segment:1 left main stem (cofficient 5) and 4 right branch segment (proximal coefficient 2.5, middle coefficient 1.5, distal coefficient 1, right posterior descending branch coefficient 1),5 left anterior descending branch segment (proximal coefficient 2.5, middle coefficient 1.5, distal coefficient 1, the first and second opposite angle branch coefficient 0.5). The CHD scores is the summation of stenosis degree multipuled corresponding coefficient of each patient.we defined highest score as the left main stem completelly blockage (32×5=160) and define no pathological changes as 0 so was divide into 3 steps [normal,middle(1-80 score),high (81-160 score)] to quantitate the coronary heart disease.
