| To evaluate the values of study using 1.5T MRI scaning in the normal bone marrow and normal lumbar disks in adults and patients with malignant bone metastases in MR-DWI and WB-DWI and common MRI/CT. To assess the significant of MR-DWI and WB-DWI+MRI/CT in detecting normal bone marrow and patients with malignant bone metastases.All of the 90 pantients (55 healthy volunteers and 35 patients with malignant bone metastases) were enrolled in this study.30 healthy volunteers underwent routine MRI and DWI, and 25 of healthy volunteers underwent WB-DWI+MRI/CT,35 patients with malignant bone metastases underwent WB-DWI, MRI and CT in one week. Bone metastases were verified either by pathological results or MR/CT findings or follow up more than five monthes. All the scan data of WB-DWI was and processed by Functool and 3D-MIP software. The statistical analysis of the relevant parameters was performed by SPSS 13.0. The ADC values of normal bone marrow and normal lumbar disks differ with age and sex were calculated (27 subjects younger than 50 years old group 1 and 28 subjects older than 50 years old group 2). The sensitivity, accuracy and specificity of WB-DWI+ MRI/CT and routine MRI/CT were calculated.The ADC values of lumbar vertebral bone marrow, ilium and femur (55 healthy volunteers) were (0.438±0.077)×10-3mm2/s, (0.304±0.056)×10-3mm2/s, (0.297±0.062)×10-3mm2/s. The stastic show:the value of lumbar vertebral bone marrow were higher ilium and femur (p< 0.001). There were no significant difference in ilium and femur. The correlation of ADC of lumbar vertebral bone marrow on MR-DWI in adults with age and sex were:the value of ADC in group 1 were higher than those in group 2 (p< 0.0001). In group 1, the value of ADC in female subjects were higher than group 2 (p< 0.05). In group 2, there was no significant difference in the value of ADC between female subjects and male subjects (p>0.05). The value of ADC in female subjects in group 1 were higher than those in female subjects in group 2 (p< 0.05). There was no significant difference in the value of ADC between male subjects in groupl and male subjects in group 2 (p> 0.05). There was negative correlation beween the value of ADC and age in female subjects (p< 0.0001). There was no correlation between the value of ADC and age in male subjects (p> 0.05). The value of lumbar disks in adults were (1.66±0.18)×10-3mm2/s. The value of lumbar disks in lower 50 ages were (1.71±0.19) x10-3mm2/s. The value of lumbar disks in higher 50 ages were (1.31±0.11)x 10-3mm2/s. The value of lumbar disks were gradually decreased with ages. The values of bone metastases tumor were (1.611±0.564)×10-3mm2/s. There are significant usefulness between the normal bone marrow and malignant bone metastasis (p< 0.0001). The sensitivity, specifity and accuracy for detecting the malignant bone metastasis were 97.22%,61%,87.24% respectively by routine MRI/CT in combination with WB-DWI, and 70.37%,61%,67.79% respectively by routine MRI/CT. So there was significant difference in diagnosing malignant bone metastases between two methods (p< 0.05).The study of ADC values about the normal bone marrow apply good basic and guidance for DWI in clinic advanced application and degeneration of vertebral column with aging higher, WB-DWI+MRI/CT is a rapid, efficient and practical method in detecting bone metastasis tumor and has much more sensitivity and accuracy and a significant value in clinical application comparing with MRI/CT. |
