Objective:To analyse the clinicopathological features of FAP and advantages and disadvantages of different surgical methods. Compare total colorectal resection ileal S-pouch anal anastomosis with J-pouch in complications, stool forming time, daily stool frequency, fecal incontinence comprehensive score, anal sensation etc and explore reasonable diagnosing and treating methods.Methods:The clinicopathologic data of 58 patients with FAP hospitalized from April 1995 to November 2009 in GuangXi Medical University First Affiliated Hospital were collected. Follow-ups were taken by telephone and letter. Clinicopathological features, survival time, surgical complications and postoperative continence function were analysed.Results:58 cases,45males and 13females, aged 32.51±10.61(16~54) were analysed.29 patients had clear family history of FAP,15 patients had suspicious family history and 13 patients not.The main clinical manifestations included bloody stool, stool character,habits change, abdominal pain or abdominal discomfort. Synchronous malignant changes were found in 26 patients while hospitalized. Malignant sites were mainly located in the sigmoid colon and rectum(74.9%).54 patients were performed operation.43 patients were performed colectomy ileal pouch anal anastomosis(27 S-pouch and 16 J-pouch). 6 patients were performed total colorectal resection and permanent ileostomy and 4 patients without surgery. There was no operative mortality and totally complication rate was 16.3%.S-pouch and J-pouch showed no striking difference in postoperative complications, stool forming time, daily stool frequency, fecal incceontinence comprehensive score, anal sensation etc.Conclusions:FAP had no specific clinical manifestations and was easily cancerate. Colonoscopy is the most important diagnostic procedure.Proved cases' family screening is quite impotant.Surgery is the most important treatment.There is not striking difference of postoperative complications and postoperative continence function between total colorectal resection ileal S-pouch anal anastomosis and J-pouch anal anastomosis.
