Objective To investigate the effect of different dose sufentanil and fentanyl on blind nasotracheal intubation. Methods Sixty ASAⅠⅠpatients,aged 1676 years, weight 4075kg, elective oral and maxillofacial surgery were randomly allocated to three groups(n=20 each): groupⅠ( sufentanil 0.2μg/kg) groupⅠ( sufentanil 0.3μg/kg),groupⅢ(fentanyl 2μg/kg) .Anesthesia was induced with midazolam 0.1 mg / kg and followed by sufentanil 0.2,0.3μg/kg or fentanyl 2μg /kg respectively.SBP,DBP,HR and SPO2 were measured and recorded before induction (T0), just before intubation(T1), immediately after intubation(T2), 5minutes(T3) and 10minutes(T4) after intubation. The venous blood samples were taken to measure the blood serum adrenaline(AD) concentrations of before induction (T1), immediately after intubation (T2) and 5minutes after intubation (T4). Result When compared with baseline value, SBP was declined just before intubation, immediately after intubation the SBP has raised 5minutes and 10 minutes after intubation has declined in turn (P < 0.05). When just before intubation DBP declined significantly, immediately after intubation elevated in, 5minute after intubation falls and 10 minute after intubation tends to stabilize, The change and the difference was statistically significant compared with basic value(P<0.05). Three groups'SBP and DBP change trend is consistent and the peak value does not exceed basis values. GroupⅠHR changes compared to basic value other heart rate value decline in turn(P<0.05)groupⅠHR change compare to baseline value just before intubation HR has risen, T2, T3, and T4 point decline in turn(P<0.05), groupⅢ, compared with baseline value, just before intubation HR was dropped, immediately after intubation HR has risen, but not more than basic value,5minute after intubation and 10 minute after intubation the HR are down(P<0.05). AD concentration: Compared with baseline value,after intubation AD concentration increased significantly(P<0.05). Three groups of SBP,DBP,HR and AD concentration difference was no statistically significant (P > 0.05). The incidence of respiratory inhibition in order groupⅢ> groupⅠ>groupⅠ. Only one case occurred respiratory inhibition in groupⅠ(5%),six cases occurred respiratory inhibition in groupⅠ(30%),groupsⅢhave seven cases occurred respiratory inhibition(35%). One case has occurring a transient chest wall muscular rigidity in groupⅠduring induction, but restore automatically.Conclusion Sufentanil 0.2μg/kg can be applied to the sedation and analgesia in blind nasotracheal intubation completely, which with lower side-effects.
