Objective to investigate adenosine deaminase(ADA),Lactic acid(LA),Lactate dehydrogenase(LDH) and C-reactive protein(CRP) activity in cerebralspinal fluid for differential diagnosis of three type meningitis in acute stage. Using Receive Operating Characteristic(ROC) curve to compare the diagnosis value of ADA,LA,LDH,CRP of tuberculous meningitis and purulent meningitis .Mhthods Using continuous monitoring method,end-point method,immune turbidimetry and continuous monitoring method by automatic biochemistry analyzer, the level of ADA,LA,LDH,CRP in cerebrospinal fluid were measured in 26 cases of viral meningitis ,20 of tuberculous meningitis, 21 of purulent meningitis, and compared with 22 control cases.. we drew the ROC curves of ADA,LA,LDH,CRP in tuberculous meningitis group and purulent meningitis group and calculate area under the curve(AUC) ,95%confidence interval(CI) cut of point value(COV),sensitivity and specificity.Results compared with control cases ,there were significantly elevated ADA,LA,LDH,CRP level in purulent meningitis, tuberculous meningitis(P<0.05) and no significant differences in viral meningitis(P>0.05);the purulent meningitis group had remarkably higher LA,LDH,CRP level than tuberculous meningitis and viral meningitis(P<0.05);compared viral meningitis group , the tuberculous meningitis group had remarkably higher ADA,LA,LDH,CRP level(P<0.05). in the tuberculous meningitis group, ADA level was significantly higher than purulent meningitis and viral meningitis(P<0.05).Conclusion the ADA,LA,LDH,CRP activity in cerebralspinal fluid is favourable for the early diagnosis and treatment of three type meningitis. The diagnostic values is significantly when the level of ADA is≥7.8U/L in the tuberculous meningitis group, CRP≥19.5mg/L,LDH≥26.5 U/L in cerebralspinal fluid for the diagnosis of purulent meningitis might be valuable .
