Objective: To investigate the expression of TGF-β1, CTGF, P-JNK,α-SMA and FN intubulointerstitial of patients with IgA nephropathy (IgAN) and to analyze the effect of JNK onrenal tubular epithelial cells transdifferentiation and the expression of CTGF, In order to furtheranalyze the relationship of JNK and renal interstitial fibrosis, to offer the theoretical supports forpathogenesis, condition assessment and preventional measure of IgAN.Methods: Refering classification of Haas type about pathomorphology of IgAN , Thirty-ninerenal biopsy specimen of patients with IgAN were divided into mild proliferationgroup(MP,twelve cases,Haas,Ⅱtype), focal proliferation group(FP,fourteen cases,HaasⅢtype)and proliferation-sclerosis group (PS,thirteen cases,HaasⅣ,Ⅴtype). Set up five cases for controlgroup. Observe the pathologic changes in tubulointerstitial of experiment groups and controlgroup by light microscope, the score of tubulointerstitial lesion(TIL) was according to Katafuchisemi-quantitative analyses. TGF-β1, CTGF, P-JNK,α-SMA and FN in renal tubulointerstitialwere detected by immunohistochemistry staining and analyzed the significance of these indicesand the relationships with TIL.Results: The score of TIL and the expression of TGF-β1, CTGF, P-JNK,α-SMA and FNincreased in the groups of IgAN, besides the score of TIL andα-SMA in the MP and the controlgroup hadn't obvious difference(P>0.05), the difference was remarkable between other groups(P<0.01). Among them, The expression of CTGF, P-JNK,α-SMA and FN increasedsignificantly with the type of IgAN getting serious(P<0.01); The expression of TGF-β1 in PSwas lower obviously compared with MP and FP(P<0.01). The pearson correlation analysisshowed that the expression of TGF-β1,α-SMA, FN and the score of TIL was positivelycorrelated with the expression of P-JNK (r=0.35*,0.82,0.89;*P<0.05,others P<0.01); Theexpression of TGF-β1 and the score of TIL was positively correlated with the expression ofα-SMA (r=0.64,0.83;P<0.01); The expression of P-JNK, TGF-β1,α-SMA and the score of TILwas positively correlated with the expression of CTGF (r=0.90,0.42*,0.93, 0.87, 0.91;*P<0.05,others P<0.01).Conclusion: 1. The expression of TGF-β1 was positively correlated with the expression ofα-SMA, the expression of TGF-β1,α-SMA, FN and the score of TIL was positively correlatedwith the expression of P-JNK, it hints that JNK involves in regulating the TGF-β1-duced transdifferentiation of renal tubular epithelial cells and involves in the development oftubulointerstitial fibrosis in patients with IgAN.2. The expression of P-JNK, TGF-β1,α-SMA, FN and the score of TIL was positively correlatedwith the expression of CTGF, the expression of TGF-β1, CTGF, FN and the score of TIL waspositively correlated with the expression of P-JNK, it clews that CTGF participates in theTGF-β1-induced myofibroblast proliferation and extracellular matrix deposition, JNKinvolves in the TGF-β1-duced expresstion of CTGF in renal tubular epithelial cells andinvolves in the development of tubulointerstitial fibrosis in patients with IgAN.3. The expression of P-JNK, CTGF andα-SMA in tubulointerstitial increase significantlywith the type of IgAN getting serious, it hints that P-JNK, CTGF andα-SMA maybeimportant indices to reflect the pathogenetic condition and prognosis of IgAN.
