Objective: To estimate the economic burden of posthepatitic cirrhosis and hepatocellular carcinoma patients per year in Taiyuan Shanxi province,and explore its influencing factors.Methods: The Department of Digestive of the first hosipital of Shanxi medical uinivercity and the third hospital of Taiyuan were choosed.The samples were randomly selected with cluster sampling method.Using a questionnaire investigated each patient's direct economic costs, indirect expenses and intangible economic cost, according to health economics evaluation method, discussed the economic burden and analyzed by SPSS13.0.Results: We investigated 149 hospitalized patients who were diagnosed of posthepatitic cirrhosis and hepatocellular carcinoma in Taiyuan Shanxi provence. The compensatory cirrhosis patients' direct economic burden annual per capita was 38036.21yuan, including for drug costs 12762.90 yuan,indirect expenses were 4396.01yuan, direct:indirect ratio 8.7:1,intangible economic burden was 48891.89 yuan.The total finacial burden made up about 267.1 persent of household income, 539.2 persent of personal annual income, 206.9 persent of GDP per capita of Taiyuan; The lose- compensatory cirrhosis patients' direct costs were 46756.49yuan, drug costs were 12828.53yuan, indirect and intangible expenses were 4618.55yuan and 59448.28yuan respectively, direct:indirect ratio 10.1:1. The total finacial burden made up about 315.2 persent of household income, 559.7 persent of personal annual income, 251.1 persent of GDP per capita;The primary liver cancer patients' was 57256.03yuan, 7861.30yuan and 147187.50yuan respectively, direct:indirect ratio 7.3:1. The total finacial burden made up about 608.8 persent of household income, 1029.4 persent of personal annual income, 481.1 persent of GDP per capita. Single factor analysis shows that, there were statistical differences between the household register effect on intangible costs, direct costs, indirect expenses and total cost (p < 0.05). Professions to intangible cost, direct cost and the influence of total cost had statistically difference (p < 0.05).Multivariate regression analysis showed, hospital days was the common influencing factors of direct and indrect economic burden(t=5.008/3.920/2.929,p<0.05), professional and personal income had greater influence on intangible costs(t=-2.417/3.596,p<0.05).Conclusion: The total expenses of all patients with liver disease were more than twice as much as per capita income, 5 times more of the family income, and accounts for more than 200% GDP of Shanxi Province, not only to bring a heavy burden to patients and their family, also brings to the society. And as the illness grew worse, expenses also increased, direct medical costs were far outweigh the direct non-medical expenses, direct costs were much more than indirect expenses.The government should further increase investment,make relevant policy and improve madical environment,so as to reduce the financial burden of the disease.
