| Pulmonary arterial hypertension, that always complicated with chronic obstructive pulmonary disease (COPD), is the key to turning into cor pulmonale. If Pulmonary arterial tension is controlled timely, Symptoms and prognosis would be improved. Recently, with research of pulmonary hypertension making progress rapidly, drugs of treating pulmonary hypertension has been making remarkable progress. Sildenafil, a target drug of pulmonary arterial hypertension, has made significant progress. in treatment of primary pulmonary hypertension (IPAH).Due to the COPD with pulmonary hypertension and IPAH have similar pathophysiological mechanism, so we will discussed in this article about clinical curative effect of sildenafil in treating COPD with pulmonary hypertension。Objective:To discuss about clinical curative effect of sildenafil in treating COPD with pulmonary hypertension。Method:50 cases of COPD with pulmonary hypertension patients were hospitalized in the First Hospital of Jilin University from January 2009 January 2011, and all of the patients is meeting requirements of internal diagnostic criteria, NYHA II～III assessed as cardiac function. We randomly divides them into group A (treatment group) and group B (control group), and each group are 25 cases. Before the treatment, all patients should stop taking medication for Lowering blood pressure, then mesure 6 minutes walking distance, pulmonary artery pressure,Blood pressure and heart rate, blood oxygen saturation,blood oxygen pressure and blood carbon dioxide pressure and the function of liver and kidney. Group A (treatment group), based on the conventional treatments, add with oral with 50mg of sildenafil, three times daily, and treatment 12 weeks; Group B (control group) is with conventional treatment, such as oxygen therapy, control of infection, eliminating phlegm, spasmolysis etc. After treating for 12 weeks, then mesure 6 minutes walking distance,pulmonary artery pressure,Blood pressure,heart rate, blood oxygen saturation,blood oxygen pressure and blood carbon dioxide pressure and the function of liver and kidney again。Results:pulmonary contraction tension of the treatment group is 89.3+ 5.3 mmHg before treating, and reduced to78.9+5.5mmHg, pulmonary contraction tension of the control group is 90.6+6.3 mmHg before treating, and reduced to 85.9+6.2mmHg, the drops degree of the treatment group is larger than the control group's (P<0.05)。Six minutes walking distance of the treatment group is255.7±46.5m before treating, and increase to368.7±35.5m, Six minutes walking distance of the control group is 259.7±36.5m before treating, and increase to 299.7±28.5 m, the increase degree of the treatment group is larger than the control group's (P<0.05)。Blood oxygen pressure of the treatment group is67.2±5.3mmHg before treating, and increase to 72.1±5.8mmHg, Blood oxygen pressure of the control group is 66.5±4.1mmHg before treating, and increase to 69.2±5.1mmHg, the increase degree of the treatment group is larger than the control group's (P<0.05)。Blood oxygen pressure of the treatment group is85.3±3.9%before treating, and increase to 89.6±4.37%, blood oxygen pressure of the control group is 85.1±5.0% before treating, and increase to 87.3±4.5%, the increase degree of the treatment group is larger than the control group's (P<0.05)。FEV1% of the treatment group is 60.3±5.3% before treating, and increase to 65.7±3.5%, blood carbon dioxide pressure of the control group is 61.5±4.6% before treating, and increase to63.1±5.1%, the increase degree of the treatment group is larger than the control group's (P<0.05)。Changes of Blood carbon dioxide pressure,Blood pressure,the function of liver and kidney were not statistically significant (P>0.05).Conclusion:Sildenafil can reduce pulmonary artery pressure, improve 6 minutes walking distance,increase the blood pressure and blood oxygen saturation。The side effect of blood pressure,FEV1/FVC,kidney and liver function, blood pressure is small。Sildenafil is a relatively safe drug and need larger clinical trials for further proved.
