Background:Thoracic trauma plays a very important role in both wartime and peacetime, and it has a high death rate. Therefore, it is of great significance in both wartime and peacetime to strengthen the research on war thoracic trauma and to improve its diagnostic and treatment level.Since the 1990s, video-assisted thoracic surgery (VATS) has been applied to the diagnosis and treatment of thoracic trauma. However, there are still limitations. The thoracoscope under local anesthesia features smaller wound and more convenience, but it also has many defects. This Research is going to combine thoracic closed drainage with thoracoscopy under local anesthesia, in order to make the minimally invasive conditions reach their maximum extent, and at the same time, to realize quicker diagnosis and treatment of the traumatic conditions. The Research takes pigs and dogs as the experimental subjects. It is intended to verify its feasibility and practicability and safety in thoracic closed drainage operations and fiber thoracoscopy under local anesthesia.Objective:Design and make a thoracic closed drainage tube for field operations; conduct preliminary research on its functions in experimental pigs and dogs, and verify its practicability and safety.Methods:Design and make a thoracic closed drainage tube for field operations;Take adult pigs as the experimental animals. Conduct operations of thoracic closed drainage and fiber thoracoscopy under local anesthesia on them;Take adult mongrel dogs (6 dogs) as the experimental animals. Monitor their breath, cardiac rate, blood pressure, blood oxygen saturation, oxygen partial pressure and other indicators after anesthesia, before operation, and during open pneumothorax or controlled closed pneumothorax; and keep done the records.ResuIts:The purpose-made thoracic closed drainage tube for field operations can facilitate thoracic closed drainage operations and conventional thoracoscopy under local anesthesia;Statistically, there is no noticeable difference (P>0.05) among the data of cardiac rate, diastolic pressure and respiratory rate under the following circumstances:before operations and during open pneumothorax or controlled closed pneumothorax; but there are significant differences in that of systolic pressure (P<0.05), which demonstrates higher pressure when open pneumothorax breaks out; there are also significant differences in the aspects of blood oxygen saturation and oxygen partial pressure (P<0.05), which are lower when open pneumothorax breaks out.Conclusion:The application of purpose-made thoracic closed drainage tube for field operations can provide convenience for the thoracic closed drainage and thoracoscopy under local anesthesia. It is applicable to substitute the special flexible trocar of LTF-240 thoracoscope and to mitigate the operation impacts upon the physiology of the experimental dogs. Therefore, it has prominent application prospects.
