Objective:The aims of this experiment are to observe the expression and localization of tumor necrosis factor alpha(TNF-a)of granulation tissue and cholesteatoma in chronic otitis media,and explore its relationship with chronic otitis media.methods:Use of efficient ready-to-immunohistochemistry MaxVisionTM2 method to detect the expression of TNF-a in 24 cases of middle ear granulation tissue,24 cases of middle ear cholesteatoma,16 cases of external auditory canal and 10 cases of normal external ear canal skin.Results:1. TNF-a was localized in the cytoplasm, brownish yellow granules, some nuclear staining. In the cholesteatoma tissue, the positive staining in the epithelial layer has a full-color, the strongest is the cuticle coloring. In the granulation tissue, the positive cells have the whole layer of coloring, the major part of the capillary and the coloring of inflammatory cells. Ear canal skin in 4 cases (40%) showed weak positive staining, uniformly distributed in the full-thickness skin,6 specimens were negative.2. The expression of TNF-a in Middle ear and external auditory canal is higher than the middle ear granulation tissue.3. Middle ear, the positive cell rate and average optical density of TNF-a are respectively(84.09±10.14)% and 0.973±0.084; External auditory canal cholesteatoma, the positive cell rate and average optical density of TNF-a are (72.26±8.26)% and 0.933±0.109;Significantly higher than in the granulation tissue TNF-a positive cells (56.94±8.99)% and the average optical density 0.798±0.073. Both TNF-a positive cells in the rate and the average optical density compared with the granulation tissue were significant differences(P all<0.05).4. Either cholesteatoma or granulation tissue, higher the average optical density of TNF-α,more stronger the bone destruction will be. This shows that TNF-a and the middle ear bone destruction is related.Conclusions:1. The expression of TNF-a is detected in both cholesteatoma and granulation tissue, and the expression of TNF-a in cholesteatoma is higher than in granulation tissue.2. In Chronic otitis media both cholesteatoma and granulation tissuean can cause the bone destruction, the extent of bone destruction is related with the degree of tumor necrosis factor-a expression, the stronger of TNF-a expression, the more severe of bone destruction, that is a positive correlation.3. Cholesteatoma of external auditory canal can also cause the bone destruction, the extent of bone destruction is also correlated with the level of tumor necrosis factor-a expression.
