Drug does not only harm addicts' own physical and mental health, but also endanger the stability of the family, hinder the social development and threaten the health of human beings, for which detoxification has become hot discussed in medical research around the world. Methadone maintenance treatment (MMT) is an alternative treatment method for the dependence of opioid drug. It is also a replacement therapy in case that the eradication of drug addiction is not achieved. During the MMT, there are still craving for heroin, smoking, and relapse. In addition, methadone itself is opioid narcotic analgesics. Compared with heroin, methadone still has lower euphoria and dependence. After long-term usage, the withdrawal of patients who depend on methadone treatment is the same difficult as heroin withdrawal treatment of patients; for withdrawal treatment of drug addicts lies in the desire for drugs is an objective, no matter MMT or detoxification treatment can not eradicate this desire. The core of complete and successful treatment for patients with heroin dependence is to reduce craving and put an end to relapse and absorption phenomenon. Therefore, how to reduce patients' craving during MMT and help them keep away from the drug and return to society is of far-reaching social significance.ObjectiveIt is a worldwide problem that MMT has high smoking and relapse rate and is difficult to eradicate drugs completely. This thesis adopts heroin craving scale, methadone dosage and the positive urine test to observe the changes of patients'Craving level in MMT before and after treatment of acupuncture. The patients treated by acupuncture plus MMT are adopted as the experimental group, while the patients treated by MMT alone as the control group. They were compared before and after acupuncture treatment to observe the improvement of MMT patients' craving desire clinically, and to explore new therapies for prevention and treatment of this disease, providing simple and effective clinical treatments.MethodsThis thesis adopts a randomized controlled single-blind method to observe the MMT patients, who are all from Xiamen Siming Cheung Hong methadone maintenance treatment clinics. Patients are selected based on a random number table and divided into two groups of 50 cases randomly. There is no intervention in the case of the original MMT, patients of acupuncture plus MMT treatment were taken as the experimental group, while patients of simple MMT as the control group. Patients in the experimental group are treated by "Jin's Three Needles" therapy in the treatment of encephalopathy for the common four points, namely four points for Shen, three points for stabilization, three points on occipital and three points for intelligence on hand, combined with the dialectical points. The two MMT groups follow the provisions of the original program. The experimental group is treated by acupuncture six times from Monday to Saturday weekly. According to the original MMT programs, the control group is treated a month for a period of treatment. The heroin craving score scale, methadone dosage, and positive urine test are observed before and after treatment. Then database package SPSS11.5 is used to make logical proof. In order to make the two groups comparable, comparison and analysis between the two groups for gender, age, education level, occupation, drug use, drug abuse period, amount of heroin per day before MMT, the positive rate of urine are conducted. The enumeration data of the two groups are analyzed by chi-square, the dose data by t-test and ranked data by rank sum test.Results1. The writer of this thesis adopts statistical tool to analyze the craving scores of the two groups before and after treatment and finds that the scores has statistically significant differences (P<0.05). Besides, the experimental group is better than the control group, with statistical significance (P<0.05). Experimental group improved mainly in drug craving intention, desire for drugs and expectation, with statistical significance (P<0.05). However, there is no significant effect (P>0.05) in self-control. The craving of the control group improved mainly in terms of drug desire with statistically significance (P<0.05), but no statistically significance (P>0.05) in self-control, medication intention and the expectation of effect.2. After statistical processing of methadone dosage of the two groups before and after treatment, it is found that the difference of the experimental group before and after treatment was statistically significant (P<0.05), but there is no significant difference control group before and after treatment (P> 0.05).3. There is no statistical significant difference of the positive urine test for the two groups before and after treatment (P> 0.05).Conclusion1 Acupuncture can improve patients'the craving in the methadone maintenance treatment, especially in drug intentions, drug desire, expectation of effects, and the decrease of the amount of methadone.2 In this research, three points for stabilization, three points on occipital and three points for intelligence on hand of "Jin's Three Needles" therapy are adopted to observe the improvement of craving for MMT patients of the experimental group, which has achieved satisfactory efficacy and is proved to own broad clinical application.
