Endoscopic Ultrasonography (EUS) Diagnosis And Endoscopic Treatment Of Early Gastric Cancer

Objective:To evaluate the diagnostic and therapeutic values on early carcinoma of stomach by Endoscopic ultrasonography (EUS) and endoscopic submucosal dissection(ESD)/endoscopic mucosal resection(EMR).Methods:42 cases early carcinoma of stomach that diagnosed by EUS and pathology were selected, and with the help of EUS, EMR/ESD was performed on early carcinoma of stomach cases., which were selected as endoscopic treatment group. The resected specimens were sent to take histlolgical examination, and the patients were regularly endoscopic followed-up after operation.At the same period,36 cases early carcinoma of stomach in surgery were selected as operation treatment group. To observe the accuracy of EUS and security of EMR/ESD in endoscopic treatment group. Results:The accuracy of EUS judged early carcinoma of stomach was 92.8%, the accuracy of m cancer and sm cancer was respectively 92.3%,93.1%. With the help of EUS, the success rate of EMR/ESD was 97%. Most patients had abdominal pain; a few had ulceration after EMR/ESD. No other complications such as perforation were found.1 case upper gastrointestinal bleeding was found,2 cases recurrence was found during the follow-up of 5 years. Compared the operation treatment groups, there were shortened in the aspects of operation time, hospital stay, there were lower complications, average survival time hadn't different significantly (p>0.05). Conclusion:EUS has a high accurate rate in diagnosis of early carcinoma of stomach, and EMR/ESD is not only a complete, reliable, and safe method to treat early carcinoma of stomach, it has but also been superior of safe, low hurt, low cost, low complication, attain the similar effect with surgical.
