Objective: To analyze the clinical features and investigate curative effect of various surgical treatments for the patients who suffer from lumbar intervertebral disc herniation complicated with lumbar instability.Methods: Retrospective investigation including general information, curative effect and feature of various operative programs. Research methodology is divided into: (1) According to whether or not the basis of surgical posterior pedicle screws fixation and spinal fusion, the patients were divided into non-internal fixation group and internal fixation group. The internal fixation group is divided into the single segment group and plural segments group according to the number of segments fixed. (2)Those patients who have gotten the treatment of surgical posterior pedicle screw fixation were treated with spinal fusion simultaneously. The spinal fusion group is divided into pure graft group and Cage graft group according to the way of bone graft.Results: All the patients were followed up and 22 cases have got excellent effects, 10 cases have got good effects according to the JOA score. Operative time and hemorrhage of the discectomy non-internal-fixation is less than the group of the discectomy with spinal fusion by internal-fixation. Internal fixation group shows more effective than non-internal-fixation group. The operative effect is the same between the single segment group and the plural segments group. The operative effect of the Cage graft group is better than the pure graft group's.Conclusion: Once the diagnosis of lumbar disc herniation complicated with segmental lumbar instability is definite,the surgical posterior pedicle screw fixation and spinal fusion operation should be perform in the meantime.
