Objective: Hypertensive disorders complicating pregnancy are peculiar happened at women's diseases duration of pregnancy, including the health of the mother and baby to a large degree, and it is one of the major reasons why the pregnant and estational hypertension, preeclampsia, eclampsia, pre-eclampsia hypertension and chronic hypertension. Severe pre-eclampsia influencesing in women and perinatal fall ill and die. Until now, cause and pathogensis are, however, not yet clear. According to the results of recent clinical and basic experiments it is generally accepted that the disease may relate to genetic predisposition, immunological maladjustment, ischemia of the placenta and oxidative stress reaction. The uterus pathophysiology expression of patients catching with severe pre-eclampsia are placenta hypoperfusion, decreased perfusion and placental infarction, concretion in helicinoartery increased protein fibrin precipitation. Moreover, cruor and imbalance of anticoagulation are also important pathophysiologic changes showing pre-eclampsia.Pleiotrophin(PTN)is multifunctional factor of the same family with MK. Another name for PTN is heparin binding factor which is related to vascularization, tissue repair, the growth of bone, kidney and lung, mitosis activity, antiapoptotic activity, activity associated with cancer, accelerating the growth activity of nerve cells differentiation and axon and relevant activity of senile dementia. Recently, some researchers put forword that PTN can get the dopaminergic neuron to differentiation and influence the metabolism catecholamine substances. PTN can combine with heparin and affects the balance of cruor and anticoagulation system, that is related to the pathology feature of anticoagulation system imbalance of severe pre-eclampsia. It can be referred that PTN has some relations as a result with the pathogensis of severe pre-eclampsia. But studies about PTN and severe pre- eclampsia are fairly few, therefore the specific mechanism is not yet known either.Syndecand-1 is a member of transmembrane adhesion molecule integrin binding proteoglycans . Syndecand-1 can combine with various factors, and take part in the regulation of physiological process such as development and differentiation of histoorgan, vascularization, fissne regeneration, etc. It also relates to the homing and shifting of tumor cells and judges the indicators of some tumor prognoses. Some researches show that syndecan-1 can combine with antithrombinâ…¢(AT-â…¢) through proteoglycan of ectodomain and join the anticoagulation process. Whether or not it has some relationships with the imbalanced cruor and anticoagulation system of the patients with severe pre- eclampsia .Catechol-O-methyltransferase (COMT) is a major digestive enzyme in the metobolic process of Catecholamine, and it degrade Catecholamine by catalyzing the methylation of catecholamine located at position3 hydroxyl group. And its activity is decided by genetic factors with the influence of hormonal level. Lately, Subhasis Banerjee has done some researches on COMT by using animal models lacking of comt gene, showing that little mice lacking of comt gene all reduced to get pre-eclampsia. Then, whether or not the comt gene in blood circulation and placenta will get the same change with human patients and take part in the pathogenesis of severe pre- eclampsia. The article investigates whether or not PTN, COMT and Syndecand-1 are meaning for to severe pre- eclampsia by studying the expression of PTN, COMT and Syndecand-1 protein of the placenta of normal pregnancy and patients getting severe pre- eclampsia. How there three factors work and whether they are related to each other will provide theoretical principle for pathogen of severe pre- eclampsia and clinical treatment.Methods:The group of severe pre-eclampsia: choosing 15 patients getting pre- eclampsia who are examed and hospitalized for childbirth at The Affiliated Hospitalof Hebei medical college from September, 2009 to March, 2010. The age of these pregnant women:24-29 years. Pre-eclampsia is diagnosed and classified according to women's health (published in the sixth edition by people's health press). The group of normal pregnancy: choosing 15 normal pregnant women for of the same period in our hospital, pregnancy and the age of these pregnant women:24-29years,and the average gestational week:34- 39weeks. Pregnant women in both of the two groups are of singleton pregnancy, with no history of repeated abortion and no assistance from the reproductive techniques. They are also free from medicine and surgery diseases such as pregnancy associated with hypertension, gestation diabetes, pregnancy associated with diabetes, pregnancy associated with cardiac disease etc. All the study objects end their gestation by cesarean section.After the placental expulsion, it's necessary to collect explant in the size of 1*1*1 cm in just the middle of the generant, then to absorb the blood with dry gauze. Next, the experimenter should put it in the PV tube, and make a mark on the tube, infusing some liquid nitrogen to keep it freezed. All the steps are done in a sterile environment. Then, the sample is put in a frige of -70 centigrade preparing for use. The experimente use Western blotting to examine Protein content Of PTN sydecan-1and COMT in placentabetweenthe normal group and the severe pre-eclampsia group. All the statis- ticsare analysed by the method of SPSS13.0.Two samples are compared by t-test, and take P<0.05 as the significance level.Result:Severe pre-eclampsia group and nomal group placentas PTN,COMT and Syndecand-1 protein detection, there was a significant difference between the two groups and severe pre-eclampsia group were lower than the normal control group.Conclusions:PTN ,COMTand Syndecand-1 protein may have all taken part in the outburst of the severe pre-eclampsia. That can provide theoretical principle for finding pathogenesis of the disease and some bases for clinical treatment to follow.Whether or not there is a kind of action way among PTN,COMT and Syn decand-1 and through this way how they work together on the outburst of severe pre-eclampsia. But how they work together needs further study. That will be of great importance to the treatment of severe pre-eclampsia.
